血清PECAM-1、PLGF水平联合子宫动脉多普勒超声参数检测在妊娠高血压患者不良妊娠结局中的预测价值

摘　　要：目的探究子宫动脉多普勒超声参数、血清血小板内皮细胞黏附分子-1(PECAM-1)、胎盘生长因子(PLGF)联合检测对妊娠高血压(HDCP)患者不良妊娠结局的预测价值。方法选取2022年09月-2023年08月于我院就诊的65例HDCP患者作为观察组,同期选取65例健康产检者作为对照组进行研究,比较2组及不同程度HDCP患者子宫动脉多普勒超声参数[脐动脉搏动指数(PI)、阻力指数(RI)、血流速度峰谷比(S/D)]及血清PECAM-1、PLGF水平,通过比较不同妊娠结局的子宫动脉多普勒超声参数及血清PECAM-1、PLGF水平,绘制受试者工作特性(ROC)曲线分析预测价值。结果研究组RI、PI、S/D均高于对照组,且不良妊娠结局者RI、PI、S/D高于良好妊娠者(P<0.05);研究组及不良妊娠结局者血清PECAM-1、PLGF水平均明显低于对照组及良好妊娠结局者(P<0.05);HDCP组血清PECAM-1、PLGF水平高于轻度子痫前期组,PI、RI、S/D低于轻度子痫前期组;重度子痫前期组血清PLGF、PECAM-1水平低于轻度子痫前期组,RI、PI、S/D高于轻度子痫前期组(P<0.05);血清PECAM-1、PLGF水平联合多普勒超声参数(子宫动脉)预测HDCP患者不良妊娠结局的AUC为0.916,95%CI为0.820~0.970,均高于单独诊断。结论血清PECAM-1、PLGF水平、子宫动脉多普勒超声参数联合检测在预测HDCP疾病不良妊娠结局中具有较高的预测价值,临床意义较高。Objective To investigate the predictive value of combined detection of uterine artery Doppler ultrasound parameters,serum platelet endothelial cell adhesion molecule 1(PECAM-1)and plavental growth factor(PLCF)in adverse pregoancy oulcomes in patients with pregnarney-induced hypertension(HDCP).Methods A total of 65 patients with HDCP who were admitted to our hospital from September 2022 to August 2023 were selected as the observation group,and 65 healthy obstetricians were selected as the control group during the same period.Uterine artery Dopplerultrasound parameters[umbilical artery pulsation index(PI),resistance index(RI),peak-to-valley ratio(S/D)of blood flow velocity]and serum PECAM-1 and PLGF levels were compared between the two groups and patients with different degrees of HDCP.By comparing uterine artery Doppler ultrasound parameters of different pregnancy outcomes and serum PECAM-1 and PLCF levels,Receiver operating characteristic(ROC)curve was plotted to analyze the predictive value.Results RI,PI and S/D in the study group were higher than those in the control group,and RI,PI and S/D in the poor pregnancy outcome group were higher than those in the good pregnancy(P<0.05).Serum PECAM-1 and PLGF levels in the study group and those with adverse pregnancy outcome were significantly lower than those in the control group and those with good pregnancy outcome(P<0.05).Serum PECAM一1 and PLCF levels in HDCP group were higher than those in mild preeclampsia group,and PI,RI and S/D levels were lower than those in mild preec lampsia group.Serum levels of PLGF and PECAM-1 in severe preclampsia group were lower than those in mild preclampsia group,and RI,PI and S/D were higher than those in mild preeclampsia group(P<0.05).Serum PECAM-1 and PLGF levels combined with Doppler ultrasound parameters(uterine artery)predicted the AUC of adverse pregnancy outcomes in HDCP patients to be 0.916 and 95%CI to be 0.820-0.970,which were higher than those diagnosed alone.Conclusions The combined detection of serum PECAM-1,PLGF and
