预见性结合综合保温护理在腹腔镜下全子宫切除手术患者中的应用

摘　　要：目的:探讨预见性结合综合保温护理在腹腔镜下全子宫切除手术(TLH)患者中的应用.方法:选取2020年1月至2023年1月期间103例在本院接受TLH手术患者为研究对象.依照护理方案,将其分为观察组(n=53,给予预见性结合综合保温护理)和对照组(n=50,给予常规护理).比较两组患者各时间段体温变化、术后麻醉苏醒相关指标、术后凝血功能.结果:观察组患者术中1h、术中2h及术毕体温均高于对照组,差异均有统计学意义(P<0.05).观察组患者拔管时长、完全苏醒时长均短于对照组,且心率高于对照组,寒战率低于对照组,差异均有统计学意义(P<0.05).观察组患者凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、D-二聚体(D-D)水平均低于对照组,差异均有统计学意义(P<0.05).结论:预见性结合综合保温护理在TLH手术患者中的应用表现出显著优势,不仅能够有效维持患者术中体温稳定,还有助于提高患者苏醒质量,纠正可能出现的凝血功能紊乱.Objective:To explore anticipatory combined with integrated thermal care in total laproseopie hysterectomy(TLH)surgery.Methods:A total of 103 patients who underwent TLH surgery between January 2020 and January 2023 at our hospital were selected as study subjects.According to the nursing programme,the patients were divided into observation group(n=53,received anticipatory combined with comprehensive insulation care)and control group(n=50,received conventional care).Temperature changes,postoperative anaesthesia awakening related indexes,postoperative coagulation functionwerecompared between the two groups in each time period.Results:The intraoperative 1h,intraoperative 2h and end-of-surgery body temperature of observation group were higher than those of control group,and the differences were statistically significant(P<0.05).The length of extubation and the length of complete awakening of observation group were shorter than those of control group,the heart rate was higher than that of control group,and the chilling rate was lower than that of control group,and the differences were statistically significant(P<0.05).Prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT)and D-dimer(D-D)levels in observation group were lower than those in control group,and the differences were statistically significant(P<0.05).Conclusion:The application of anticipatory combined integrated thermal insulation care in TLH surgical patients has shown significant advantages,which not only can effectively maintain stable intraoperative body temperature of the patients,but also help improving the quality of patients'awakening,correct possible coagulation disorders.
