摘 要:目的 探讨肋骨接骨板内固定、胸带外包扎固定两种治疗方法在多发性肋骨骨折(Fracture of Multiple Ribs, FMR)患者中的应用价值。方法 回顾性选取2022年1月—2023年1月浙江省人民医院毕节医院收治的106例FMR患者的临床资料,根据治疗方法不同分为观察组与对照组,每组53例。对照组采用胸带外包扎固定,观察组采用肋骨接骨板内固定,比较两组骨折恢复情况、用力肺活量(Forced Vital Capacity, FVC)、每分钟最大通气量(Minute Ventilation Volume, MVV)、动脉血氧分压(Arterial Partial Pressure of Oxygen, PaO_(2))、血氧饱和度(Arterial Oxygen Saturation, SaO_(2))、不良反应发生率。结果 观察组住院时间为(12.51±2.98)d,短于对照组的(13.92±3.80)d;观察组下床活动时间为(7.46±1.90)d,短于对照组的(8.57±2.06)d;观察组骨折愈合时间为(2.50±0.65)个月,短于对照组的(2.84±0.62)个月,差异有统计学意义(t=2.126、2.884、2.756,P均<0.05)。观察组FVC、MVV、PaO_(2)水平高于对照组,不良事件发生率明显低于对照组,差异有统计学意义(P均<0.05)。结论 与胸带外包扎比较,采用纯钛六爪抓握式接骨板内固定治疗FMR,能够进一步纠正血气分析指标,加速骨折恢复,对患者的肺部功能有一定的改善作用。Objective To explore the application value of rib plate internal fixation and chest bandage external fixation in patients with fracture of multiple ribs(FMR).Methods The clinical data of 106 patients with FMR admitted to Zhe-jiang Provincial People's Hospital Bijie Hospital from January 2022 to January 2023 were retrospectively selected.Ac-cording to different treatment methods,they were divided into observation group and control group,with 53 cases in each group.The control group was fixed with external bandage of chest bandage,and the observation group was fixed with rib plate internal fixation.The fracture recovery,forced vital capacity(FVC),minute ventilation volume(MVV),arterial partial pressure of oxygen(PaO_(2)),arterial oxygen saturation(SaO_(2))and the incidence of adverse events were compared between the two groups.Results The hospitalization time of the observation group was(12.51±2.98)d,which was shorter than(13.92±3.80)d of the control group;the time of getting out of bed in the observation group was(7.46±1.90)d,which was shorter than(8.57±2.06)d in the control group;the fracture healing time of the observation group was(2.50±0.65)months,which was shorter than(2.84±0.62)months of the control group,and the differences were statistically significant(t=2.126,2.884,2.756,all P<0.05).The levels of FVC,MVV and PaO_(2) in the observation group were higher than those in the control group,and the incidence of adverse events was significantly lower than that in the control group,the differences were statistically significant(all P<0.05).Conclusion Compared with the chest bandage external fixation,the treatment of FMR with pure titanium six-claw grasping rib plate internal fixation can further correct the blood gas analysis index,accelerate the recovery of fracture,and improve the lung function of patients.
