白内障超声乳化、人工晶体植入、房角分离联合术式治疗闭角型青光眼与传统小梁切除术的临床效果对比

摘　　要：目的探讨闭角型青光眼患者采用白内障超声乳化、人工晶体植入、房角分离术联合术式的临床治疗效果。方法便利选取2020年10月—2023年1月于邵武市立医院就诊的66例闭角型青光眼患者为研究对象,根据手术方式的不同分为传统组(n=33)和综合组(n=33),综合组患者接受白内障超声乳化、人工晶体植入、房角分离术联合术式治疗,传统组患者接受传统小梁切除术治疗,对比两组患者的裸眼视力、眼压和前房深度水平、术后不良反应发生情况和复发情况。结果术前,两组患者的裸眼视力、眼压、前房深度比较,差异无统计学意义(P均>0.05);术后3个月,综合组裸眼视力和前房深度水平高于传统组,眼压低于传统组,差异有统计学意义(P均<0.05)。综合组术后不良反应发生率为9.09%,低于传统组的15.15%,但两组比较,差异无统计学意义(χ^(2)=0.569,P>0.05)。综合组复发率为3.03%,低于传统组的18.18%,差异有统计学意义(χ^(2)=3.995,P<0.05)。结论白内障超声乳化、人工晶体植入、房角分离术联合术式相较于传统小梁切除术治疗闭角型青光眼,能更好地改善视力水平、眼压和前房深度,同时安全性较好且复发率低。Objective To explore the clinical effects of combined cataract phacoemulsification,intraocular lens implantation and angle separation surgery in patients with angle closure glaucoma.Methods A total of 66 patients with angle-closure glaucoma treated in Shaowu Municipal Hospital from October 2020 to January 2023 were conveniently selected as the study objects,and were divided into traditional group(n=33)and comprehensive group(n=33)according to different surgical methods.Patients in the comprehensive group received cataract phacoemulsification,intraocular lens implantation,and angle separation combined treatment.Patients in the traditional group received traditional trabeculectomy.The naked eye vision,intraocular pressure,anterior chamber depth level,incidence of postoperative adverse reactions,and recurrence rate of the two groups of patients were compared.Results Before operation,there was no significant difference in naked eye visual acuity,intraocular pressure and anterior chamber depth between the two groups(all P>0.05).Three months after operation,the naked eye visual acuity and anterior chamber depth in the comprehensive group were higher than those in the traditional group,and the intraocular pressure was lower than that in the traditional group,the difference were statistically significant(all P<0.05).The incidence of postoperative adverse reactions in the comprehensive group was 9.09%,which was lower than that in the traditional group(15.15%),but there was no significant difference between the two groups(χ^(2)=0.569,P>0.05).The recurrence rate of the comprehensive group was 3.03%,which was lower than that of the traditional group(18.18%),and the difference was statistically significant(χ^(2)=3.995,P<0.05).Conclusion Compared with traditional trabeculectomy,cataract phacoemulsification,intraocular lens implantation and angle separation can improve visual acuity,intraocular pressure and anterior chamber depth better,with better safety and lower recurrence rate.
