手术与保守治疗不稳定型桡骨远端骨折的临床效果评估

摘　　要：目的评估手术与保守治疗在不稳定型桡骨远端骨折中的临床效果。方法方便选取2021年1月—2023年9月金沙县中医医院收治的106例不稳定型桡骨远端骨折患者为研究对象。遵循随机数表法分为两组,对比组(n=53)采取保守治疗,实践组(n=53)采取手术治疗。对比两组患者腕关节功能、掌倾角、尺偏角、骨折愈合时间、治疗有效率、并发症发生情况。结果治疗前,两组患者腕关节功能、掌倾角、尺偏角对比,差异无统计学意义(P均>0.05);治疗后,实践组腕关节功能优于对比组,掌倾角、尺偏角均大于对比组,差异有统计学意义(P均<0.05)。实践组骨折愈合时间短于对比组,差异有统计学意义(P<0.05)。实践组治疗有效率为98.11%,高于对比组的84.91%,差异有统计学意义(χ^(2)=4.371,P=0.037)。实践组并发症发生率低于对比组,差异有统计学意义(P<0.05)。结论不稳定型桡骨远端骨折患者应用手术治疗可提高治疗有效率,改善腕关节功能与尺偏角,加快骨折愈合,且并发症较少。Objective To evaluate the effect of surgical and conservative treatment of unstable distal radius fractures.Methods 106 patients with unstable distal radius fractures treated in Jinsha County Hospital of Traditional Chinese Medicine from January 2021 to September 2023 were conveniently selected as the study objects.They were divided into two groups according to the random number table method,the control group(n=53)received conservative treatment,and the practice group(n=53)received surgical treatment.The wrist joint function,palmar inclination,ulnar deviation,fracture healing time,treatment efficiency and complications were compared between the two groups.Results Before treatment,there was no significant difference in wrist joint function,palmar inclination angle and ulnar deviation angle between the two groups(all P>0.05);after treatment,the wrist joint function of the practice group was better than that of the comparison group,and the palmar inclination angle and ulnar deviation angle were greater than those of the comparison group,the differences were statistically significant(all P<0.05).The fracture healing time of practice group was shorter than that of comparison group,and the difference was statistically significant(P<0.05).The effective rate of the practice group was 98.11%,higher than that of the comparison group(84.91%),and the difference was statistically significant(χ^(2)=4.371,P=0.037).The incidence of complications in practice group was lower than that in comparison group,and the difference was statistically significant(P<0.05).Conclusion Surgical treatment of unstable distal radius fractures can improve the effective rate,improve the wrist joint function and ulnar angle,accelerate fracture healing,and have fewer complications.
