脑外伤后脑水肿予以吡拉西坦注射液联合甘露醇治疗的效果评价

摘　　要：目的探讨脑外伤后脑水肿患者治疗期间应用吡拉西坦注射液联合甘露醇方案的临床价值。方法随机选取2022年6月—2023年12月滨州市第二人民医院收治的84例脑外伤后脑水肿患者为研究对象。以奇偶分组法将患者分为参照组和研究组,每组42例。参照组患者行甘露醇治疗,研究组患者应用吡拉西坦注射液联合甘露醇治疗。对两组患者的血清学指标、影像学指标、临床效果进行分析。结果治疗后,研究组的神经功能评分低于参照组,认知功能评分高于参照组,差异有统计学意义(P均<0.05)。治疗后,研究组脑水肿量(2.32±0.20)mL、水肿体积(2.41±0.24)mL均低于参照组,差异有统计学意义(t=49.513、33.191,P均<0.05)。治疗后,研究组神经元特异性烯醇化酶、水通道蛋白4相关血清学指标结果均低于参照组,差异有统计学意义(P均<0.05)。结论吡拉西坦注射液联合甘露醇治疗对改善脑外伤后脑水肿患者神经功能作用显著,并且有助于缩小脑水肿体积。Objective To investigate the clinical value of piracetam injection combined with mannitol in the treatment of patients with cerebral edema after traumatic brain injury.Methods A total of 84 patients with cerebral edema after traumatic brain injury admitted to Binzhou Second People's Hospital from June 2022 to December 2023 were randomly selected as the study subjects.The patients were divided into reference group and study group by add-even grouping method,with 42 cases in each group.The patients in the reference group were treated with mannitol,and the patients in the study group were treated with piracetam injection combined with mannitol.The serological indexes,imaging indexes and clinical effects of the two groups were analyzed.Results After treatment,the neurological function score of the study group was lower than that of the reference group,and the cognitive function score was higher than that of the reference group,the differences were statistically significant(both P<0.05).After treatment,the cerebral edema volume was(2.32±0.20)mL,edema volume was(2.41±0.24)mL in the study group,which were lower than those in the reference group,and the differences were statistically significant(t=49.513,33.191,both P<0.05).After treatment,the results of neuron-specific enolase and aquaporin 4 related serological indexes in the study group were lower than those in the reference group,the differences were statistically significant(all P<0.05).Conclusion Piracetam injection combined with mannitol has a significant effect on improving the neurological function of patients with cerebral edema after traumatic brain injury,and it is helpful to reduce the volume of brain edema.
