摘　　要：目的:探讨改良式封闭负压辅助闭合技术联合湿性敷料在肠外瘘患者中的应用效果。方法:选取2018年12月—2021年12月于宜宾市第一人民医院普外科行手术治疗的91例肠外瘘患者作为研究对象,随机分为观察组(n=42)、对照组(n=49)。对照组采用常规对症治疗,观察组在对照组基础上采用改良式封闭负压辅助闭合技术联合湿性敷料治疗。比较两组瘘口周围皮炎发生情况、日均换药次数、自发性愈合率及瘘口愈合时间。结果:与对照组比较,观察组瘘口周围皮炎发生率更低、日均换药次数更少、瘘口自发性愈合率更高、自发愈合时间更短,差异均具有统计学意义(P<0.05)。结论:改良式封闭负压辅助闭合技术联合湿性敷料在肠外瘘患者中的应用效果良好,可有效控制患者瘘口周围皮炎的发生,减少换药次数,促进瘘口愈合。Objective:To explore the application effect of modified closed negative pressure-assisted closure technique combined with wet dressing in patients with extraintestinal fistula.Methods:Ninety-one patients with extraintestinal fistula who underwent surgical treatment in the Department of General Surgery of Yibin First People's Hospital from December 2018 to December 2021 were selected as the study subjects,and were randomly divided into the observation group(n=42)and the control group(n=49).The control group was treated with conventional symptomatic treatment,and the observation group was treated with modified closed negative pressure-assisted closure technology combined with wet dressings on the basis of the control group.The occurrence of perifistula dermatitis,the average number of daily dressing changes,the spontaneous healing rate and the healing time of fistula were compared between the two groups.Results:Compared with the control group,the observation group had a lower incidence of perifistula dermatitis,a lower average daily number of dressing changes,a higher spontaneous healing rate of fistula,and a shorter spontaneous healing time,and the differences were all statistically significant(P<0.05).Conclusion:The application of modified closed negative pressure-assisted closure technique combined with wet dressing in patients with extraintestinal fistula is effective in controlling the occurrence of perifistula dermatitis in patients,reducing the number of dressing changes,and promoting the healing of fistula.
