摘　　要：为了探索运动转诊在社区初级卫生保健体系的实践运行,基于国内外研究、实践经验和实地访谈发现:体卫融合内容尚未纳入基层医疗卫生机构评价,经费筹措缺乏依据,行政隶属复杂、分工不明、跨部门协同不力,标准建设与质量监控滞后等诸多问题。研究在确立体卫融合连续体的基础上,对运动转诊内部运转机制、外围周边体系两个方面进行论述,确定运动转诊协调人的角色、职责,并就有关从业人员的资质进行厘定,将体力活动水平纳入生命体征、运动处方作为“药物”,运动干预过程参照传统临床治疗路径,从操作层面提出适宜目前经济社会发展的运动转诊运行流程,解决运动处方实际执行问题,突出运动转诊周边配套标准、支付、保险、监管等保障要素的重要性,为运动转诊纳入社区初级卫生保健体系提供环境支持。In order to explore the practical operation of exercise referral schemes(ERS)in the primary health care system,based on domestic and foreign research,practical experience and on-site interviews,it was found that the integration of physical and health services has not yet been included in the evaluation of community-level medical and health care institutions,there is a lack of basis for funding,complex administrative affiliation,unclear division of labor,weak cross departmental collaboration,and lagging standard construction and quality monitoring.Based on the establishment of a continuum of physical and health integration,it aims to discuss the internal operating mechanism and external linkage system of ERS,determine the roles and responsibilities of ERS coordinators,and determine the qualifications of relevant practitioners,incorporate physical activity levels into vital signs,use sports prescriptions as“drugs”.The process of sports intervention follows traditional clinical treatment pathways for sports intervention,from an operational perspective,proposes a suitable process for ERS operation in the current economic and social development context,solves the problem of actual execution of exercise prescriptions,highlights the importance of supporting standards,payment,insurance,supervision,and other security elements for ERS,and provides environmental support for incorporating ERS into the primary health care system.
