腹腔镜经腹腹膜前疝修补术对青年患者睾丸血管及生殖功能的影响

摘　　要：目的:探讨腹腔镜经腹腹膜前疝修补术对青年患者睾丸血管及生殖功能的影响。方法:纳入2018年6月至2020年1月行腹腔镜经腹腹膜前疝修补术的32例男性患者作为研究对象,分别于术前1 d及术后1个月、6个月进行超声检查,测量患者双侧精索静脉内径、睾丸体积、睾丸动脉内径、睾丸动脉收缩期峰值血流速度等指标,并常规检测外周血清睾酮浓度、国际勃起功能指数。结果:32例手术均获成功。术后1个月,患侧精索静脉管径较术前明显增粗(P<0.05);术后6个月,测量管径与术前相比差异无统计学意义(P>0.05)。术后1个月、6个月,患者外周血清睾酮浓度、国际勃起功能指数、睾丸体积、睾丸动脉内径、睾丸动脉收缩期峰值血流速度与术前相比差异无统计学意义(P>0.05)。结论:腹腔镜经腹腹膜前疝修补术后早期可影响患侧精索静脉内径,术后6个月即可恢复正常,治疗青年腹股沟疝是安全、可靠的,不会影响患者的睾丸血管及生殖功能。Objective:To explore the influence of the transabdominal preperitoneal inguinal hernia repair on the testicular vessels and reproductive function of youth patients.Methods:There were 32 male patients with inguinal hernia enrolled in this study who were treated with laparoscopic transabdominal preperitoneal hernia repair from Jun.2018 to Jan.2020.All patients underwent ultrasound examination one day before surgery,one month and six months after surgery for bilateral spermatic vein inner diameter,testicular vo-lume,testicular artery inner diameter,the peak systolic blood flow velocity of testicular artery.And the serum testosterone concentrations as well as the international index of erectile function were monitored and compared.Results:The 32 patients were all successfully operated.Compared with before operation,the spermatic vein diameter was significantly enlarged 1 month after operation.However,there was no significant difference between the data measured 6 months after operation and the preoperative data.There were no significant diffe-rences in serum testosterone concentration,international erectile function index,testicular volume,testicular artery inner diameter and peak systolic blood flow velocity before operation,1 month and 6 months after surgery.Conclusions:The laparoscopic transabdominal preperitoneal inguinal hernia repair may affect the inner diameter of spermatic vein in the early postoperative period,and the diameter could restore normal after 6 months.This procedure is safe and reliable,and has no significant influence on the testicular vessels and r eproductive function of patients.
