胎宝汤治疗黄体功能不全型先兆流产疗效研究

摘　　要：目的:探究胎宝汤对治疗黄体功能不全型先兆流产患者的应用价值。方法:选取黄体功能不全型先兆流产患者72例,采用信封随机抽取法分为对照组(n=36)和治疗组(n=36),对照组给予地屈孕酮治疗,治疗组给予胎宝汤联合地屈孕酮治疗。比较两组疗效、中医症状评分、症状缓解时间、血清同型半胱氨酸(Hcy)和内分泌激素水平[血绒毛膜促性腺激素(β-HCG)、孕酮(P)、雌二醇(E2)]及不良反应。结果:治疗组临床总有效率(97.22%)高于对照组(80.56%)(P<0.05);治疗后治疗组中医症状评分低于对照组(P<0.05);治疗组腹痛、腰酸消失时间、阴道止血时间及孕酮恢复时间均短于对照组(P<0.05);治疗后两组Hcy均下降,且治疗组低于对照组;β-HCG、P及E2均上升,且治疗组高于对照组(均P<0.05);对照组和观察组不良反应总发生率分别为16.67%和8.33%(P>0.05)。结论:胎宝汤治疗黄体功能不全型先兆流产疗效佳,可有效缓解临床症状,促进血清内分泌激素水平升高,Hcy水平降低,且安全性良好。Objective:To explore the application value of Taibao decoction in the treatment of patients with luteal insufficiency type of threatened abortion.Methods:72 patients with luteal insufficiency type of threatened abortion were classified into control group(n=36)and treatment group(n=36)by adopting random envelope sampling method.The control group received dydrogesterone treatment,whereas the treatment group was treated with Taibao decoction combined with dydrogesterone.The efficacy,TCM symptoms scores,symptom relief time,serum homocysteine(Hcy)and endocrine hormones[blood chorionic gonadotropin(β-HCG),progesterone(P)and estradiol(E2)]and adverse reactions were compared.Results:The treatment group(97.22%)had total clinical effective rate than control group(80.56%)(P<0.05).The TCM symptoms scores were lower in treatment group after treatment(P<0.05).The abdominal pain relief time,disappearance time of waist soreness,vaginal hemostasis time and progesterone recovery time in treatment group were shorter compared with those in control group(P<0.05).After treatment,Hcy level was declined in both groups,and the Hcy level in treatment group was lower.The levels ofβ-HCG,P and E2 were enhanced,and the treatment group had higher levels(P<0.05).The total incidence rates of adverse reactions in control group and treatment group were 16.67%and 8.33%(P>0.05).Conclusion:Taibao decoction has good efficacy in the treatment of luteal insufficiency type of threatened abortion.In addition,it can effectively relieve the clinical symptoms,promote the increases of serum endocrine hormones,and reduce Hcy level,and it has good safety.
