微小RNA-492、红细胞微粒水平与ST段抬高型急性心肌梗死患者冠状动脉病变严重程度关系研究

摘  要:目的:探究微小RNA-492(miR-492)、红细胞微粒(RMPs)水平与ST段抬高型急性心肌梗死(STEMI)患者冠状动脉病变严重程度的关系。方法:选取STEMI患者83例为研究对象,入院后均进行Gensini积分评分,以评估患者冠脉病变严重程度。收集患者临床相关资料,检测miR-492和RMPs水平。分析患冠状动脉病变严重程度的影响因素,以及miR-492、RMPs对患者冠状动脉病变程度的预测效能。结果:83例患者Gensini总评分为(45.38±7.21)分,其中轻度35例、重度48例。重度病变组年龄≥60岁占比、miR-492、RMPs、心肌肌钙蛋白(cTnI)、脑钠肽(BNP)、超敏C-反应蛋白(hs-CRP)水平高于轻度病变组,LVEF水平低于轻度病变组(均P<0.05)。Logistic回归分析结果显示,miR-492、RMPs、hs-CRP水平是STEMI患者冠脉病变严重程度的独立危险因素,LVEF水平是独立保护因素(均P<0.05)。Pearson法分析显示,患者miR-492、RMPs水平与Gensini评分、hs-CRP呈正相关,与LVEF呈负相关(均P<0.05)。ROC曲线分析结果显示,miR-492、RMPs及两项联合预测STEMI患者冠脉病变严重程度的敏感度分别为72.20%、77.80%、87.00%,特异性分别为75.90%、79.30%、89.70%,曲线下面积(AUC)分别为0.828、0.813、0.944(均P<0.05)。结论:miR-492、RMPs水平是影响STEMI患者冠脉病变严重程度的独立危险因素,两者联合检测可有效预测患者病变严重程度。Objective:To explore the relationship between the levels of microRNA-492(miR-492),erythrocyte microparticles(RMPs)and the severity of coronary artery lesions in patients with ST-segment elevation acute myocardial infarction(STEMI).Methods:A total of 83 patients with STEMI were retrospectively selected as the research objects.Gensini scores were performed for all patients after admission to evaluate the severity of coronary lesions.The clinical data of patients were collected,and the levels of miR-492 and RMPs were detected.The influencing factors of the severity of coronary artery disease were analyzed,and the predictive efficacy of miR-492 and RMPs on the severity of coronary artery disease in patients was analyzed.Results:The total Gensini score of 83 patients was 45.38±7.21,including 35 mild cases and 48 severe cases.The proportion of age≥60 years old,the levels of miR-492,RMPs,cTnI,BNP and hs-CRP in the severe lesion group were higher than those in the mild lesion group,and the LVEF level was lower than that in the mild lesion group(all P<0.05).Logistic regression analysis showed that miR-492 level,RMPs level,hs-CRP level were independent risk factors for the severity of coronary artery lesions in patients with STEMI,and LVEF level was independent protective factor(all P<0.05).Pearson analysis showed that the levels of miR-492 and RMPs in patients were positively correlated with Gensini score and hs-CRP,and negatively correlated with LVEF(all P<0.05).ROC curve results showed that the sensitivity of miR-492,RMPs and their combination to predict the severity of coronary artery disease in patients with STEMI was 72.20%,77.80%,87.00%respectively,the specificity was 75.90%,79.30%,89.70%respectively,and the AUC were 0.828,0.813 and 0.944 respectively(all P<0.05).Conclusion:The levels of miR-492 and RMPs are independent risk factors for the severity of coronary artery lesions in patients with STEMI.The combined detection of miR-492 and RMPS can effectively predict the severity of coronary artery lesions in patien
