摘　　要：目的分析针刺和挑刺法联合乙字汤治疗湿热下注型痔病的效果。方法选取2020年10月至2022年10月瑞金市中医院收治的60例痔病患者作为研究对象,按照随机数字表法分为观察组与对照组,每组30例。对照组给予常规治疗,观察组在对照组基础上给予针刺和挑刺法联合乙字汤治疗,比较两组临床疗效、痔病症候积分、痔核直径及排便时肛门疼痛程度[疼痛数字分级法(NRS)]、手术率及不良反应发生情况。结果观察组治疗有效率为93.33%,高于对照组的73.33%,差异有统计学意义(P<0.05);治疗后,观察组便血、肛门坠痛、肛门脱垂、痔黏膜积分均低于对照组,差异有统计学意义(P<0.05);治疗后1周及治疗结束时,观察组痔核直径短于对照组,NRS评分低于对照组,差异有统计学意义(P<0.05);两组手术率比较差异无统计学意义;两组均未出现不良反应。结论针刺和挑刺法联合乙字汤治疗湿热下注型痔病效果显著,能明显改善痔病临床症状,缓解排便时肛门疼痛,还能降低手术率,安全性较高。Objective To analyze the effect of acupuncture and picking needling combined with Yi Zi decoction in the treatment of hemorrhoids with damp-heat-injection type.Methods 60 patients with hemorrhoids admitted to Ruijin Traditional Chinese Medicine Hospital from October 2020 to October 2022 were selected as the research subjects,they were divided into the observation group and the control group according to the random number table method,with 30 cases in each group.The control group received routine treatment,while the observation group received acupuncture and pricking needling combined with Yi Zi decoction treatment on the basis of the control group.The clinical efficacy,hemorrhoid syndrome score,hemorrhoid nucleus diameter,degree of anal pain during defecation(numerical rating scale[NRS]score),surgical rate and incidence of adverse reactions were compared between the two groups.Results The effective rate of treatment in the observation group was 93.33%,which was higher than 73.33%in the control group,and the difference was statistically significant(P<0.05);after treatment,the scores of stool blood,anal pendant pain,anal prolapse and hemorrhoidal mucosa in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05);1 week after treatment and at the end of treatment,the hemorrhoidal nucleus diameter in the observation group was shorter than that in the control group,and the NRS score was lower than that in the control group,and the differences were statistically significant(P<0.05);there was no statistically significant difference in surgical rates between the two groups;there were no adverse reactions in the two groups.Conclusion Acupuncture and pricking needling with Yi Zi decoction has a significant effect in the treatment of hemorrhoids with damp-heat-injection type,can significantly improve the clinical symptoms of hemorrhoids,relieve anal pain during defecation,and reduce the surgical rate,and has high safety.
