双重超声造影联合增强CT对胃癌患者术前TNM分期的诊断价值

摘　　要：目的探讨双重超声造影(DCEUS)联合增强CT在胃癌患者术前TNM分期诊断中的价值。方法选取2020年5月至2022年7月湘潭市中心医院收治的82例胃癌患者作为研究对象,所有患者均接受DCEUS与增强CT检查,以术后病理结果为金标准,分析DCEUS、增强CT检查及DCEUS联合增强CT检查与金标准结果的一致性及诊断符合率。结果82例胃癌患者中,金标准检出TNM分期为:T1期12例,T2期10例,T3期25例,T4期35例;N0期31例,N1期29例,N2期18例,N3期4例;M0期48例,M1期34例。DCEUS检查诊断T、N、M分期均与金标准一致性良好(Kappa值为0.592、0.637、0.727);增强CT检查诊断T、N、M分期均与金标准一致性良好(Kappa值为0.412、0.714、0.729);DCEUS联合增强CT检查诊断T、N、M分期均与金标准一致性高(Kappa值0.763、0.839、0.828)。DCEUS检查对T、N、M分期的诊断符合率分别为71.95%、75.61%、86.59%;增强CT检查对T、N、M分期的诊断符合率分别60.97%、80.49%、83.33%;DCEUS联合增强CT检查对T分期的诊断符合率分别为85.36%、89.02%、92.68%。结论DCEUS联合增强CT检查应用于胃癌患者术前TNM分期的诊断中,可提高诊断效能。Objective To investigate the value of double contrast-enhanced ultrasonography(DCEUS)combined with enhanced CT in the preoperative TNM staging of patients with gastric cancer.Methods A total of 82 patients with gastric cancer admitted to Xiangtan Central Hospital from May 2020 to July 2022 were selected as the research subjects,all patients underwent DCEUS and enhanced CT examination.The postoperative pathological results were used as the gold standard,the consistency and diagnostic compliance rate of DCEUS,enhanced CT and DCEUS combined enhanced CT examination with the gold standard were analyzed.Results Among 82 patients with gastric cancer,the TNM staging of detected by gold standard was as follows:12 cases in T1 stage,10 cases in T2 stage,25 cases in T3 stage,35 cases in T4 stage;31 cases in stage N0,29 cases in stage N1,18 cases in stage N2,and 4 cases in stage N3;48 cases in M0 stage and 34 cases in M1 stage.DCEUS examination showed in the diagnosis of T,N and M stages were good consistency with the gold standard(Kappa value were 0.592,0.637,0.727,respectively).Enhanced CT examination in the diagnosis of T,N and M stages were good consistency with the gold standard(Kappa value was 0.412,0.714,0.729,respectively).DCEUS combined with enhanced CT in the diagnosis of T,N and M stages were highly consistent with the gold standard(Kappa values are 0.763,0.839 and 0.828,respectively).The diagnostic coincidence rates of DCEUS in T,N and M stages were 71.95%,75.61%and 86.59%respectively.The diagnostic coincidence rates of enhanced CT in T,N and M stages were 60.97%,80.49%and 83.33%respectively.The diagnostic coincidence rates of DCEUS combined with enhanced CT in T staging were 85.36%,89.02%and 92.68%respectively.Conclusion DCEUS combined with enhanced CT can improve the diagnostic efficiency of preoperative TNM staging in patients with gastric cancer.
