美罗培南致严重过敏样反应1例  

A Case of Severe Allergic Reaction Caused by Meropenem

摘　　要：1例输尿管结石伴严重感染的患者入院后给予美罗培南(0.5 g q8h)加强抗感染治疗,首次给药20 min后患者出现嘴唇发绀、全身寒战、四肢不自主震颤、心慌、气促等不适,BP 186/138 mmHg,P 105次/min,R 33次/min,诊断为严重过敏样反应。立即停药并给予地塞米松、呋塞米等对症治疗,40 min后患者症状逐渐缓解。经关联性分析,评价本次严重过敏样反应与美罗培南的因果关系为“很可能”。A patient with ureteral calculus complicated with severe infection was hospitalized and treated with meropenem for anti-infection therapy.The patient had cyanosis,chills,tremors,palpitations and shortness of breath at 20 minutes after the first does.The blood pressure was 186/138 mmHg,the heart rate was 105 times/min and the respiration rate was 33 times/min.The diagnosis was an severe allergic reaction.The drug was stopped and the patient was treated with dexamethasone and furosemide injections.The patient's symptoms was alleviated by symptomatic treatment after 40 minutes.By correlation analysis,the causal relationship between the severe allergic reaction and meropenem was evaluated as“very possible”.
