摘 要:目的 探讨高浓度对比剂碘美普尔400结合低毫安秒应用于头颈部CTA检查的可行性。方法 选取本院行头颈CTA检查的80例患者,随机均分为两组,对照组采用50 ml碘克沙醇(320 mgI/ml)及自动管电流;观察组采用40ml碘美普尔(400 mgI/ml)及70%自动管电流,扫描管电压及其它参数两组均一致。对图像质量进行评分,同时计算图像质量以及辐射剂量评价指标,比较所有指标的差异。结果 两组患者一般资料具有可比性(均P> 0.05)。两组增强图像血管噪声、信噪比(SNR)、对比噪声比(CNR)差异无统计学意义(t=-1.14~1.43,均P> 0.05)。观察组层面增强血管CT值高于对照组,且辐射剂量低于对照组,差异有统计学意义(P <0.001);所有图像均能满足临床诊断要求。结论 采用高浓度对比剂联合低管电流行头颈部CTA检查可在降低辐射剂量的同时保证图像质量。Objective To investigate the clinical feasibility of high concentration contrast media combined with low mAs in head and neck CT angiography(CTA).Methods A total of 80 patients who needed head and neck CTA scans at the local hos-pital were enrolled in this study and assigned into two groups randomly.The patients in control group received Iodixanol(320 mgI/ml),with an injection volume of 50 ml and automatic mAs.The patoents in observation group received Iomeprol(400 mgI/ml),with an injection volume of 40ml,whereas the effective mAs was reduced by a factor 0.7.Tube voltage and other scanning parameters were consistent between the two groups.The quality of acquired images were assessed as well as the radiation dose.Results The general information was comparable between the two groups.There was no significant difference between the two groups in image noise,SNR and CNR(t=-1.14~1.43,all P>0.05).In the observation group,the CT values of enhanced artery were significantly higher(all P<0.001)and the dose indices were significantly lower(all P<0.001)compared to the control group.All images were qualified for accurate clinical diagnosis.Conclusions The combination of increased iodine concentration and 70%mAs can further reduce the radiation dose without image quality compromise during head and neck CTA examination.
