摘 要:目的分析血清人附睾蛋白4(HE4)、干扰素-α(IFN-α)、趋化因子配体19(CCL19)、寡聚腺苷酸合成酶1(OAS1)与系统性红斑狼疮(SLE)患者疾病活动度的相关性。方法选取2022年5月至2023年5月该院收治的95例SLE患者为SLE组,根据SLE疾病活动度评分(SLEDAI)将患者分成轻度组(SLEDAI=0~<10分,40例)、中度组(SLEDAI=10~<15分,30例)和重度组(SLEDAI≥15分,25例),另选取同期100例体检健康者作为健康对照组。采用酶联免疫吸附试验检测血清HE4、IFN-α、CCL19、OAS1水平,并比较SLE组和健康对照组HE4、IFN-α、CCL19、OAS1水平,以及不同疾病活动度患者HE4、IFN-α、CCL19、OAS1水平;采用Spearman相关分析HE4、IFN-α、CCL19、OAS1与SLE疾病活动度的相关性,采用受试者工作特征(ROC)曲线分析HE4、IFN-α、CCL19、OAS1对重度SLE的诊断价值。结果SLE组HE4、IFN-α、CCL19、OAS1水平明显高于健康对照组,差异均有统计学意义(P<0.05)。血清HE4、IFN-α、CCL19、OAS1水平在健康对照组、轻度组、中度组、重度组中依次升高,两两组间比较,差异均有统计学意义(P<0.05)。HE4、IFN-α、CCL19、OAS1水平与SLE疾病活动度均呈正相关(r_(s)=0.598、0.838、0.562、0.917,P<0.05)。血清HE4、IFN-α、CCL19、OAS1单独诊断重度SLE的曲线下面积(AUC)分别为0.849、0.991、0.806、0.974,4项指标联合诊断重度SLE的AUC为0.998,大于单独检测的AUC(P<0.05)。结论血清HE4、IFN-α、CCL19、OAS1与SLE患者疾病活动度关系密切,可作为SLE疾病活动度评估的辅助指标,同时对SLE临床诊治具有重要指导意义。Objective To analyze the correlation between serum human epididymis protein 4(HE4),interferon-α(IFN-α),chemokine ligand 19(CCL19),5'-oligoadenylate synthetase 1(OAS1)and disease activity in patients with systemic lupus erythematosus(SLE).Methods A total of 95 SLE patients admitted to Pidu District People's Hospital from May 2022 to May 2023 were selected as the SLE group.According to SLE disease activity score(SLEDAI),the patients were divided into mild group(SLEDAI=0—<10 points,40 cases),moderate group(SLEDAI=10—<15 points,30 cases)and severe group(SLEDAI≥15 points,25 cases).Another 100 healthy people were selected as healthy control group.Enzyme-linked immunosorbent assay was used to detect the serum levels of HE4,IFN-α,CCL19 and OAS1.The levels of HE4,IFN-α,CCL19 and OAS1 in SLE group and control group,and in patients with different disease activity were compared.Spearman correlation analysis was used to analyze the correlation between HE4,IFN-α,CCL19,OAS1 and disease activity of SLE.Receiver operating characteristic(ROC)curve was used to analyze the value of HE4,IFN-α,CCL19,OAS1 in the diagnosis of severe SLE.Results The levels of HE4,IFN-α,CCL19 and OAS1 in SLE group were significantly higher than those in healthy control group,and the differences were significant(P<0.05).The serum levels of HE4,IFN-α,CCL19 and OAS1 in the healthy control group,mild group,moderate group and severe group increased in turn,the differences between any two groups were statistically significant(P<0.05).The levels of HE4,IFN-α,CCL19 and OAS1 correlated positively with SLE disease activity(r_(s)=0.598,0.838,0.562,0.917,P<0.05).The area under the curve(AUC)of serum HE4,IFN-α,CCL19 and OAS1 in the diagnosis of severe SLE were 0.849,0.991,0.806 and 0.974 respectively.The AUC of the combination of the 4 indicators in the diagnosis of severe SLE was 0.998,which was bigger than that of single detection(P<0.05).Conclusion Serum HE4,IFN-α,CCL19 and OAS1 relate closely to the disease activity of SLE patients,which can be us
