摘 要:目的探讨高危型急性肺血栓栓塞症(APTE)患者血清微小RNA(miR)-34a-3p和miR-106b-5p的表达和意义。方法选取该院2022年2月至2023年2月收治的85例高危型APTE患者作为研究组;选择同期在该院就诊的80例非高危型APTE患者作为对照组。采用实时荧光定量聚合酶链反应(qRT-PCR)检测所有研究对象血清miR-34a-3p和miR-106b-5p水平。采用Pearson相关分析高危型APTE患者血清中miR-34a-3p与miR-106b-5p水平的相关性。采用多因素Logistic回归分析高危型APTE发生的影响因素。采用受试者工作特征(ROC)曲线分析血清miR-34a-3p与miR-106b-5p对高危型APTE的诊断价值。结果与对照组比较,研究组血清miR-34a-3p、miR-106b-5p水平明显降低,差异均有统计学意义(P<0.05)。高危型APTE患者血清miR-34a-3p水平与miR-106b-5p水平呈正相关(r=0.764,P<0.001)。多因素Logistic回归分析结果显示,miR-34a-3p与miR-106b-5p水平升高是发生高危型APTE的保护因素(P<0.05)。血清miR-34a-3p、miR-106b-5p水平单独及二者联合检测诊断高危型APTE的曲线下面积(AUC)分别为0.810、0.772和0.828,二者联合检测诊断高危型APTE的AUC明显大于血清miR-106b-5p单独检测(Z=2.231,P=0.030),与miR-34a-3p单独检测诊断高危型APTE的AUC比较,差异无统计学意义(Z=1.156,P=0.248)。结论高危型APTE患者血清中miR-34a-3p、miR-106b-5p水平明显降低,二者可能对高危型APTE具有较高的临床诊断价值。Objective To investigate the expression and significance of serum microRNA(miR)-34a-3p and miR-106b-5p in patients with high-risk acute pulmonary thromboembolism(APTE).Methods A total of 85 patients with high-risk APTE admitted to Hebei Petro China Central Hospital from February 2022 to February 2023 were selected as the study group.During the same period,80 patients with non-high-risk APTE admitted to the hospital were selected as the control group.Real-time fluorescent quantitative polymerase chain reaction(qRT-PCR)was used to detect the serum levels of miR-34a-3p and miR-106b-5p in all subjects.Pearson correlation was used to analyze the correlation between serum miR-34a-3p level and miR-106b-5p level in high-risk APTE patients.Multivariate Logistic regression was used to analyze the influencing factors of high-risk APTE.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic value of serum miR-34a-3p and miR-106b-5p for high-risk APTE.Results Compared with the control group,the serum levels of miR-34a-3p and miR-106b-5p in the study group decreased significantly,and the differences were statistically significant(P<0.05).The level of serum miR-34a-3p correlated positively with the level of miR-106b-5p in high-risk APTE patients(r=0.764,P<0.001).Multivariable Logistic regression analysis results showed that increased miR-34a-3p and miR-106b-5p levels were protective factors for high-risk APTE(P<0.05).The area under the curve(AUC)of the serum miR-34a-3p,miR-106b-5p levels separately and the joint detection diagnosis for high-risk APTE were 0.810,0.772 and 0.828 respectively.The AUC of the joint detection diagnosis for high-risk APTE was bigger than the AUC of serum miR-106b-5p detection alone(Z=2.231,P=0.030),there was no statistically significant difference on AUC between the joint detection and miR-34a-3p detection alone diagnosing for APTE(Z=1.156,P=0.248).Conclusion The serum levels of miR-34a-3p and miR-106b-5p decreased significantly in high-risk APTE patients,and they may have h
