骨膜牵张技术配合中药治疗糖尿病足患者的效果

摘　　要：目的:分析骨膜牵张技术配合中药治疗糖尿病足患者的效果。方法:选取2021年1月—2023年10月建瓯市立医院收治的97例糖尿病足患者作为研究对象,依据治疗方法不同将患者分为对照组(常规疮疡贴湿化再生治疗,n=49)和观察组(骨膜牵张技术配合中药治疗,n=48),比较两组溃疡面愈合情况、溃疡大小、溃疡深度、经皮氧分压、踝肱指数、足部皮温、疼痛程度、神经病变情况、生活质量。结果:治疗后,观察组溃疡面愈合率、经皮氧分压、踝肱指数、足部皮温、生活质量评分高于对照组,溃疡大小、溃疡深度、视觉模拟评分法(visual analogue scale,VAS)评分、密歇根神经病变筛查表(Michigan neuropathy screening instrument,MNSI)评分、10 g尼龙丝试验评分低于对照组,差异有统计学意义(P<0.05)。结论:与常规疮疡贴湿化再生治疗相比,骨膜牵张技术配合中药治疗糖尿病足患者效果更好,可提高溃疡面愈合率,缩小溃疡面积,促进局部微循环,减轻疼痛,改善神经病变,提高生活质量。Objective:To analyze the effect of periosteal stretch technique combined with traditional Chinese medicine in the treatment of patients with diabetic foot.Method:A total of 97 patients with diabetic foot who treated in Jian’ou Municipal Hospital from January 2021 to October 2023 were selected as the observation objects,the patients were divided into the control group(conventional ulcer plaster humidification and regeneration treatment,n=49)and the observation group(periosteal stretch technique combined with traditional Chinese medicine treatment,n=48)according to the different treatment methods.The ulcer surface healing,ulcer size,ulcer depth,transcutaneous oxygen partial pressure,ankle brachial index,foot skin temperature,pain degree,neuropathy,quality of life were compared between two groups.Result:After treatment,the healing rate of ulcer surface,transcutaneous oxygen partial pressure,ankle brachial index,foot skin temperature and quality of life score in the observation group were higher than those in the control group,while the ulcer size,ulcer depth,visual analogue scale(VAS)score,Michigan neuropathy screening instrument(MNSI)score and 10 g nylon filament test score were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Compared with conventional ulcer plaster humidification and regeneration treatment,periosteal stretch technique combined with traditional Chinese medicine is more effective in the treatment of patients with diabetic foot,it can improve the healing rate of ulcer surface,reduce the ulcer area,promote local microcirculation,relieve pain,improve neuropathy and improve the quality of life.
