炎症损伤在高血压靶器官损害中的作用及中医药治疗研究进展

摘　　要：高血压引起的靶器官损害所导致的左心室肥厚、心力衰竭、慢性肾脏病、脑卒中、动脉粥样硬化等并发症严重危害人类身体健康,研究表明炎症损伤为高血压靶器官损害作用机制之一,贯穿于心、肾、脑、动脉等靶器官损害全过程。近年来,在整体观念、辨证论治和治未病等中医基础理论指导下,中医药对于防治高血压靶器官损害有良好的应用前景。中医药具有多靶点、多系统、个体化、不良反应小、安全性高等治疗优势,能发挥控制血压、抗炎、保护血管内皮功能等作用以干预高血压靶器官损害,是防治高血压靶器官损害的有效措施。中医认为高血压靶器官损伤属本虚标实,治宜扶正祛邪,灵活运用滋阴、益气、活血、化痰、祛风、清热之法。炎症损伤为诱导高血压靶器官损害的关键介质,通过中医药干预和调节炎症因子水平及炎症反应,对于改善高血压靶器官损伤进程具有重要意义。从中药复方、中药单方到中药单体的实验证实,许多中药具有通过调节炎症因子、炎症信号通路抑制炎症反应,从而发挥改善高血压靶器官损害的作用。故文章从炎症损伤角度阐述其与高血压靶器官损害相关性及中医药治疗策略,为临床上通过中医药调节炎症损伤以防治高血压靶器官损害提供理论依据。Left ventricular hypertrophy,heart failure,chronic kidney disease,stroke,atherosclerosis and other complications caused by target organ damage caused by hypertension seriously endanger human health.Studies have shown that inflammatory damage is one of the mechanisms of target organ damage of hypertension,and inflammatory reaction runs through the whole process of target organ damage of hypertension,such as heart,kidney,brain and artery damages and so on.In recent years,under the guidance of the holistic concept,syndrome differentiation and treatment,preventive treatment of disease and other traditional Chinese medicine theories,TCM has a good application prospect for the prevention and treatment of hypertension target organ damage.Traditional Chinese medicine has the advantages of multi-target,multi-system,individualization,small adverse reactions and high safety.It can play the role of controlling blood pressure,anti-inflammatory,and protecting vascular endothelial function and other effects to intervene the target organ damage of hypertension.It is an effective measure to prevent and treat the target organ damage of hypertension.Traditional Chinese medicine believes that the target organ damage of hypertension belongs to deficiency of origin and excess of symptoms,which should be treated by strengthening the body and eliminating pathogenic factors,and flexibly using the methods of nourishing Yin(阴),benefiting Qi(气),promoting blood circulation,resolving phlegm,expelling wind and clearing heat.Inflammatory injury is the key mediator to induce hypertension target organ damages.TCM intervention and regulation of the level of inflammatory factors and inflammatory response is of great significance in improving the process of hypertension target organ damage.Experiments of TCM compounds,TCM single prescription and TCM monomer have proved that many TCM medicines can inhibit the inflammatory response by regulating inflammatory factors and inflammatory signaling pathways,thus improving the damage of target organs of
