摘　　要：目的:探讨不同年龄及月经状态女性正常乳腺表观扩散系数(ADC)值的差异及其临床意义。方法:收集2020年8月~2022年11月弋矶山医院因自觉“包块”、不适,MRI检查双侧乳腺健康受试者332例资料,根据年龄分为低年龄组(n=107)、中年龄组(n=114)、高年龄组(n=111)。根据不同月经状态将受试者分为月经期(n=49)、增殖期(n=77)、分泌期(n=94)和绝经后(n=112)。在西门子后处理站中测量正常乳腺ADC值,多次测量(ROI保持一致)取平均值,比较不同年龄及月经状态的乳腺ADC值差异。结果:高年龄组左右侧正常乳腺ADC值均低于中、低年龄组(P<0.05),3组左右侧正常乳腺ADC值差异均无统计学意义(P>0.05)。绝经后双侧正常乳腺ADC值均低于月经期、增殖期及分泌期(P<0.05),月经期、增殖期、分泌期及绝经后双侧正常乳腺ADC值差异均无统计学意义(P>0.05)。结论:不同年龄及月经状态下正常女性乳腺ADC值的差异及个体间的变异度较大,利用ADC值诊断女性乳腺疾病应考虑不同年龄及月经状态下的基线ADC值差异。Objective:To investigate the differences and clinical significance of apparent diffusion coefficient(ADC)value in normal mammary gland in women of different age and menstrual status.Methods:The data were collected from 332 healthy subjects undergone MRI examination in our hospital for bilateral breasts due to self-perceived“lumps”or discomfort between August 2020 and November 2022.The cases were initially divided into low-age group(n=107),middle-age group(n=114)and high-age group(n=111),and then further classified into menstrual phase(n=49),proliferative phase(n=77),secretory phase(n=94)and postmenopausal phase(n=112)by the different menstrual status.ADC of normal breasts was repeatedly measured while maintaining consistent ROIs in the Siemens post-processing workstation,and the measurements were then averaged to compare differences in breast ADC values among various age groups and menstrual statuses.Results:The ADC values of both left and right normal breasts in the high-age group were significantly lower than those in the middle-age and low-age groups(P<0.05).There was no statistical difference between the ADC value of the left and right normal breasts among the low,middle and high group(P>0.05).The ADC values of bilateral normal breasts in the postmenopausal period were significantly lower than those in the menstrual,proliferative,and secretory phases(P<0.05).There was no significant difference in the ADC values of bilateral normal breasts among menstrual,proliferative,secretory and postmenopausal phases(P>0.05).Conclusion:ADC values of normal breasts vary a lot in women of different ages and menstrual statuses,therefore,diagnosis of female breast conditions by ADC value should take the difference of baseline ADC value into consideration.
