摘　　要：目的探讨后外侧联合内侧切口治疗三踝骨折的临床疗效。方法回顾性分析2021年6月至2022年6月江阴市惠友骨科医院收治的60例三踝骨折患者的临床资料,根据手术方案分为对照组与实验组,各30例。对照组实施传统外侧入路联合内侧切口治疗,实验组实施改良后外侧联合内侧切口治疗。比较两组患者的手术时间、术后恢复情况(骨痂形成时间、负重下地时间)、术后1年的并发症(神经损伤、骨折位移、骨折延迟愈合)发生情况。结果实验组患者手术时间显著低于对照组(P<0.05);实验组患者骨痂形成时间、负重下地时间均显著短于对照组(P<0.05)。术后1年,实验组患者的并发症总发生率低于对照组,但差异无统计学意义(P>0.05)。结论后外侧联合内侧切口治疗三踝骨折,可缩短手术时间、减少术后并发症,有效促进患者术后恢复。Objective To explore the clinical efficacy of the combined posterolateral and medial approaches for the treatment of trimalleolar fractures.Methods A retrospective study was conducted on the clinical data of 60 patients with trimalleolar fractures admitted to Jiangyin Huiyou Orthopaedic Hospital from June 2021 to June 2022.According to the surgical plan,they were divided into a control group and an experimental group,with 30 cases in each group.The control group received traditional lateral approach combined with medial incision treatment,while the experimental group received modified lateral combined with medial incision treatment.Compare the surgical time,the postoperative recovery time(callus formation time,weight-bearing time),and the total incidence of complications(nerve injury,fracture displacement,delayed fracture healing)for one year after surgery between the two groups of patients.Results The operative time in the experimental group were significantly lower than those in the control group(P<0.05);The callus formation and weight-bearing walking time in the experimental group were significantly shorter than those in the control group(P<0.05).One year after surgery,the incidence of complications in the experimental group was lower than that in the control group,but the difference was not statistically significant(P>0.05).Conclusion The combination of posterolateral and medial incision in the treatment of trimalleolar fracture can shorten the operation time,reduce postoperative complications and effectively promoting postoperative healing in patients.
