参赭镇气汤对肺肾两虚型慢性阻塞性肺病急性加重期的临床研究

摘　　要：目的:探讨参赭镇气汤治疗肺肾两虚型慢性阻塞性肺病急性加重期(AECOPD)的疗效。方法:60例AECOPD患者随机分为对照组(常规治疗)和观察组(常规治疗加参赭镇气汤),每组30例,疗程2周。比较两组临床疗效、血清炎症指标(CRP、IL-6、PCT、NLR、Cys-C)变化、疾病严重程度评分及不良反应。结果:治疗两周后,观察组患者的治疗总有效率高于对照组,差异具有统计学意义(P<0.05);两组患者胸闷气喘、咳嗽痰白、少气懒言、腰膝酸软中医证候积分与总积分均较治疗前降低(P<0.05),观察组上述证候积分与总积分均显著低于对照组(P<0.05);两组患者炎症指标均较治疗前降低(P<0.05),观察组患者CRP、IL-6、PCT、NLR、Cys-C均显著低于对照组(P<0.05);且与对照组相比,观察组患者疾病严重程度评分显著降低(P<0.05)。两组患者治疗过程中,无药物不良反应。结论:参赭镇气汤对肺肾两虚型AECOPD疗效显著,能抑制炎症、降低疾病严重程度,安全性高。Objective:To assess the efficacy of Shenzhe Zhenqi Decoction(SZD)for acute exacerbation of chronic obstructive pulmonary disease(AECOPD)with lung and kidney deficiency.Methods:Sixty AECOPD patients were randomly divided into two groups:control(conventional treatment)and observation(SZD plus conventional treatment).Results:After two weeks of treatment,The total effective rate of treatment in the observation group was higher than that in the control group,The difference between the two groups was statistically significant(P<0.05);The TCM syndrome scores and total scores of chest tightness,asthma,cough,phlegm white,low qi and lazy speech,and waist and knee soreness and weakness in both groups of patients decreased compared with before(P<0.05).The observation group had significantly lower scores and total scores than control group(P<0.05);Both groups of inflammatory indicators decreased compared to before(P<0.05),besides the observation group had significantly lower levels of CRP,IL-6,PCT,NLR,and Cys-C than control group(P<0.05);Moreover,compared with control group,the severity score of disease in observation group was significantly reduced(P<0.05).During treatment process both of the patients had no adverse drug reactions.Conclusion:SZD effectively treats AECOPD with lung and kidney deficiency,reducing inflammation and disease severity with a high safety profile.
