摘　　要：目的探讨采用主动负压帽状腱膜下引流术治疗慢性硬膜下血肿(chronic subdural hematoma,CSDH)的疗效。方法回顾性分析78例使用主动负压帽状腱膜下引流术治疗CSDH治疗效果。头颅CT检查示:单侧血肿59例,双侧血肿19例;等密度血肿45例,低密度血肿17例,混合密度血肿16例。头颅CT检查示:单侧血肿59例,双侧血肿19例;等密度血肿45例,低密度血肿17例,混合密度血肿16例。结果78例患者中有4例因血肿残留或者复发,再次钻孔置管冲洗后痊愈。无颅内感染,无脑组织损伤及脑内血肿,无癫痫发生。结论主动帽状腱膜下引流术治疗CSDH可以取得良好的手术效果,住院费用低,可以避免脑组织损伤等严重的手术并发症,是一种简便易行,安全有效,值得推广的手术方法。Objective To explore the effectiveness of active negative pressure subgaleal aponeurosis drainage in the management of chronic subdural hematoma(CSDH).Methods A retrospective analysis was conducted on 78 patients who underwent active negative pressure subgaleal aponeurosis drainage for the treatment of CSDH.Cranial CT scans showed that there were 59 cases of unilateral hematoma and 19 cases of bilateral hematoma;45 cases of equal-density hematoma,17 cases of low-density hematoma,and 16 cases of mixed-density hematoma.Results Among the 78 patients,4 cases required re-drilling and catheter irrigation due to residual or recurrent hematoma,following which they were successfully treated.No instances of intracranial infection,brain tissue damage,intracerebral hematoma,or epileptic seizures were observed.Conclusions Active negative pressure subgaleal aponeurosiscan drainage for treating CSDH demonstrates a favorable surgical outcome,with lower hospitalization cost,and reduced risk of severe surgical complications like brain tissue injury.This procedure is straightforward,secure,efficacious,and merits wider adoption.
