卵巢储备功能减退的年轻不孕症女性代谢综合征发生情况分析

摘　　要：目的探究代谢综合征(MS)在卵巢储备功能减退(DOR)的年轻不孕症女性中的发生情况并分析其影响因素。方法共纳入141名合并DOR的年轻不孕症女性(年龄<35岁),设为DOR组,另外选取141名与DOR组年龄配对且卵巢储备功能正常(NOR)的不孕症女性作为对照组(NOR组)。比较两组患者MS的发生情况并分析DOR组MS发生的影响因素。结果DOR组MS发病率明显高于NOR组,差异有统计学意义(χ^(2)=8.25,P<0.05)。logistic多因素分析发现,高FSH/LH比值为年轻不孕症女性合并DOR发生MS的独立危险因素(OR=1.26,P<0.05),而高AMH、高bLH和高bE2水平为DOR年轻不孕症女性发生MS的保护因素(OR分别=0.28、0.58、0.86,P均<0.05)。结论年轻不孕症女性中,合并DOR者MS的发病率明显升高,FSH/LH、AMH、bLH、bE2为发生MS的影响因素。Objective To investigate the incidence of metabolic syndrome(MS)in young women with infertility and diminished ovarian reserve(DOR)and analyze its influencing factors.Methods A total of 141 young women with infertility and DOR(age<35 years)were enrolled as DOR group,and another 141 young women(age-matched with DOR)with infertility and normal ovarian reserve function(NOR)group were selected as control group(NOR group).The occurrences of MS were compared between the two groups and the influencing factors were analyzed.Results The incidence of MS in DOR group was significantly higher than that in NOR group(χ^(2)=8.25,P<0.05).Logistic multivariate analysis showed that high FSH/LH ratio was an independent risk factor for MS in young women with infertility complicated with DOR(OR=1.26,P<0.05),while high AMH,basal LH and basal E2 levels were protective factors for MS(OR=0.28,0.58,0.86,P<0.05).Conclusion In young women with infertility and DOR,the incidence of MS is significantly increased.The FSH/LH ratio,AMH,bLH,and bE2 are influencing factors for MS in young women with infertility complicated with DOR.
