摘　　要：目的分析疗养机构现有国家重点监控药品使用现状,加强药品监测预警,促进临床合理用药。方法基于国家重点监控药品目录品种,对某疗养系统和门诊系统内相关药品的使用人群特点、限定日剂量、使用金额、处方点评情况以及不良反应报告情况进行统计分析。结果疗养机构国家重点监控药品类别较少,质子泵抑制剂的年度使用率最高,银杏叶提取物片、甲磺酸倍他司汀片的使用人群分别在年龄和性别上具有统计学差异;无适应证用药、存在药物相互作用以及给药剂量不适宜是不合理用药的主要影响因素;药品不良反应报告以药师报告为主,医师和护士报告比例较低,报告不够及时有效。结论后期将基于计划、执行、检查和处理(PDCA)的品管圈管理方法进行改进提升,探索形成一套适合疗养机构药品监测预警管理模式,为向康复疗养转型的临床合理用药以及其他疗养机构在药品使用管理提供经验和参考。Objective To provide data for strengthening drug monitoring and early-warning and for improving rational clinical use of drugs by analyzing the current usage of drugs under state key surveillance.Methods The characteristics of drug consumers,defined daily dose(DDS),usage,comments on prescriptions and adverse drug reactions recorded in the recuperation system and outpatient system were analyzed based on the catalogues of drugs under state key surveillance.Results Fewer types of drugs under national key surveillance were used in recuperation institutions,and proton-pump inhibitor drugs were the most frequently used.There was significant difference in age and gender between users of ginkgo biloba extract tablets and those of betahistine mesylate tablets.Medications without indications,drug interactions and inappropriate dosage were dominating practices of irrational drug use.Most of the adverse drug reactions were reported by pharmacists rather by doctors and nurses.Conclusion The quality control circle management method based on plan,do,check,action(PDCA)will be applied to opt for a model of drug monitoring and early-warning suited to recuperation institutions in order to facilitate rational use of drugs during the switch to rehabilitation.
