摘  要:目的研究推拿治疗颈型颈椎病炎性痛的镇痛机制。方法将12只兔采用随机数字表法分为空白组、模型组、推拿组,模型组和推拿组用无创颈型颈椎病动物模型,推拿组采用推拿治疗,模型组和空白组不予干预。治疗后用HE染色观察颈后肌组织形态;经酶联免疫吸附试验(ELISA)测定血清肿瘤坏死因子(TNF-α)和白细胞介素1β(IL-1β)。经RT-qPCR检测各组PI3K、AKT、mTOR通路的mRNA表达水平。结果推拿组肌纤维断裂处炎症细胞比模型组逐渐减少,肌原纤维排列整齐。模型组与空白组相比,血清TNF-α、IL-1β含量增高(P<0.01);推拿组较模型组的表达均下降(P<0.05);模型组较空白组PI3K、AKT、mTOR的mRNA表达上升(P<0.05);推拿组mRNA表达水平较模型组降低(P<0.05)。结论推拿能够降低颈型颈椎病炎性痛模型炎症因子TNF-α、IL-1β水平,可能通过降低PI3K/AKT/mTOR通路mRNA的表达发挥镇痛作用。Objective Study of the analgesic mechanism of massage therapy for the treatment of inflammatory pain in the neck type of cervical spondylosis.Methods Twelve rabbits were divided into blank group,model group and massage group by using random number table method,non-invasive the neck type of cervical spondylosis animal model was used in model and massage group,massage treatment was used in massage group,and no intervention was made in model and blank group.After treatment,HE staining was used to observe the tissue morphology of posterior cervical muscles;ELISA was used to determine serum TNF-αand IL-1β.RT-qPCR was used to detect the mRNA expression levels of PI3K,AKT,and mTOR pathway in each group.Results Inflammatory cells at muscle fibre breaks in the nudging group were gradually reduced compared with the model group,and myogenic fibres were neatly arranged.Compared with the model group and the blank group,serum TNF-αand IL-1βcontent increased(P<0.01);the expression of both decreased in the nudging group compared with that of the model group(P<0.05);the mRNA expression of PI3K,AKT and mTOR increased in the model group compared with that of the blank group(P<0.05);the level of mRNA expression in the nudging group decreased compared with that of the model group(P<0.05).Conclusions Massage was able to reduce the levels of inflammatory factors TNF-αand IL-1βin an inflammatory pain model of the neck type of cervical spondylosis,and massage may play an analgesic role by decreasing the expression of mRNA of the PI3K/AKT/mTOR pathway.
