经皮穿针治疗儿童桡骨远端骨折的临床效果分析

摘　　要：目的探讨经皮穿针治疗儿童桡骨远端骨折临床效果。方法选取2020年6月至2023年6月厦门大学附属龙岩中医院就诊的儿童桡骨远端骨折患儿60例,按随机数字表法分为对照组(30例)与试验组(30例),其中给予试验组经皮穿针治疗,给予对照组传统夹板固定治疗。对比两组患儿的骨折愈合时间、腕关节功能恢复时间、外固定拆除时间和不同骨形态(尺偏角、掌倾角、桡骨高度)等、Cooney腕关节功能评分以及可能出现的并发症。结果试验组患儿的骨折愈合时间和腕关节功能恢复时间优于对照组(P<0.05)。此外,试验组患儿腕关节尺偏角、掌倾角、桡骨高度以及Cooney腕关节功能评分等方面,与对照组相比都表现出显著改善(P<0.05)。两组病例外固定拆除时间对比无统计学差异(P>0.05)。两组患儿骨折全部愈合,无Volkmann挛缩和腕关节迟发性畸形等并发症。结论经皮穿针治疗与传统手法在治疗儿童桡骨远端骨折在骨折稳定性、腕关节功能恢复时间上优于对照组,经皮穿针治疗具有愈合快等优势,是一种安全有效的治疗方法,值得在临床中应用。Objective To explore the clinical effect of percutaneous needle puncture in the treatment of distal radius fractures in children.Methods Sixty children with distal radius fractures who visited Longyan Traditional Chinese Medicine Hospital affiliated with Xiamen University from June 2020 to June 2023 were randomly divided into a control group(30 cases)and an experimental group(30 cases)using a random number table method.The experimental group received percutaneous needle puncture treatment,while the control group received traditional splint fixation treatment.Compare the fracture healing time,wrist joint function recovery time,external fixation removal time,different bone shapes(ulnar deviation angle,palmar inclination angle,radius height),Cooney wrist joint function score,and possible complications between two groups of children.Results The fracture healing time of the test group and the recovery time of the wrist joint function are significantly better than the control group(P<0.05).In addition,the wrist ruler angle,palm angle,radial height,and Cooney wrist joint function scores in the test group have shown significantly improved compared with the control group(P<0.05).According to the comparison of fixed demolition time outside the case,there is no significant difference(P>0.05).All fractures of children in the two groups heal,without Volkmann contracture and late wrist joint deformed complications.Conclusions The treatment and traditional methods of pupil piercing and traditional methods are better than the control group in fracture stability,wrist function recovery time in the treatment of distant radius.It is worthy of promoting and applied clinical.
