摘  要:目的探讨多重磁条码液相芯片系统在抗核抗体谱检测中的应用价值。方法选取2023年10—12月深圳市第二人民医院风湿免疫科收治并被确诊为系统性自身免疫性疾病(AID)的98例患者作为AID组,另选同期进行体检且排除AID的90例健康者为对照组。分别使用多重磁条码液相芯片系统与免疫印迹法来检测两组患者血清样本中的抗核抗体和抗核抗体谱15项。比较不同检测系统的检测阳性率,并分析敏感度以及特异性等关键指标,进行严格的一致性评价。结果两种方法对AID组患者的抗核抗体阳性检出率均高于对照组(P<0.05),对抗ds-DNA抗体检测的阳性率差异具有统计学意义(P<0.05),对其余14种观测指标的检测阳性率差异无统计学意义(P>0.05)。多重磁条码液相芯片系统与金标准检测对抗Scl-70抗体检测的高度一致(Kappa≥0.75),对抗P0抗体、抗Sm抗体、抗SSA抗体、抗SSB抗体、抗CENP-B抗体、抗AMA-M2抗体、抗PCNA抗体、抗R0-52抗体、抗Histone抗体、抗Jo-1抗体检测的中度一致(0.75>Kappa≥0.40),对抗ds-DNA抗体、抗U1-snRNP抗体、抗PM-Scl抗体检测的一般一致(Kappa<0.40)。结论多重磁条码液相芯片系统与免疫印迹法检测自身抗体的总体符合率较高,但是对血清抗ds-DNA抗体的一致性不理想。对于一致性不理想的抗体,需要结合其他实验室指标、影像学检查及临床进行综合判断。Objective To explore the application value of multiplexed magnetic barcode liquid chromatography system in the detection of antinuclear antibody profile.Methods A total of 98 patients admitted to the Department of Rheumatology and Immunology of Shenzhen Second People's Hospital and diagnosed with systemic autoimmune diseases(AID)from October to December 2023 were selected as the AID group,and 90 healthy patients who underwent physical examination during the same period and excluded from AID were selected as the control group.Antinuclear antibodies and 15 items of antinuclear antibody profiles were detected in the serum samples of the two groups using a multiplexed magnetic barcode liquid chromatography system and immunoblotting,respectively.The detection positivity rates of the different detection systems were compared,and key indicators such as sensitivity as well as specificity were analysed for strict consistency evaluation.Results The positive detection rate of antinuclear antibodies of both methods for patients in the AID group was higher than that of the control group(P<0.05),and the difference in the positive rate of anti-ds-DNA antibody detection was statistically significant(P<0.05),while the difference in the positive rate of the detection of the rest of the 14 observations was not statistically significant(P>0.05).The agreement between the multiplexed magnetic barcode liquid microarray system and the gold standard test for anti-Scl-70 antibody was good(Kappa≥0.75),and consistency of anti-P0 antibody,anti-Sm antibody,anti-SSA antibody,anti-SSB antibody;anti-CENP-B antibody,anti-AMA-M2 antibody,anti-PCNA antibody,anti-R0-52 antibody,antiHistone antibody,and anti-Jo-1 antibody detection was moderate(0.75>Kappa≥0.40),and consistency of anti-ds-DNA antibody,anti-U1-snRNP antibody,and anti-PM-Scl antibody detection was poor(Kappa<0.40).Conclusions The overall compliance of the multiple magnetic barcode liquid chromatography system with immunoblotting for detection of autoantibodies was high,but the conco
