摘　　要：甲状腺危象是甲状腺毒症急性加重的一个综合征,多发生于甲亢病情未受控制的情况下,常危及生命。对需要在体外循环下进行心脏手术的甲亢患者,充分的术前准备是预防甲状腺危象的关键,术后早期识别和快速治疗甲状腺危象能够明显降低病死率,改善患者预后。本文报道1例初发甲亢的中年女性在体外循环辅助下行二尖瓣机械瓣置换术后发生甲状腺危象,经治疗后好转出院。本文分析其发生甲状腺危象可能的危险因素,为这类患者术前准备、手术时机选择及术后甲状腺危象的诊断与治疗提供一些思路。Thyroid crisis is a syndrome of acute exacerbation of thyrotoxicosis,which often occurs in the uncontrolled condition of hyperthyroidism and is often life-threatening.Adequate preoperative preparation is the key to prevent thyroid crisis in patients with hyperthyroidism who require cardiac surgery under cardiopulmonary bypass(CPB),and early postoperative recognition and rapid treatment of thyroid crisis can significantly reduce the mortality,improve the prognosis of patients.This paper reported a middle-aged woman with primary hyperthyroidism who developed a thyroid crisis after mechanical mitral valve replacement with CPB,and the possible risk factors of thyroid crisis were analyzed.This article could provide some ideas for preoperative preparation,operative timing,diagnosis and treatment of postoperative thyroid crisis.
