血清miR-223、miR-155、CCL17对小儿肺炎支原体感染的诊断价值及其与疾病严重程度的关系

摘　　要：目的探讨血清微小核糖核酸(miR)-223、miR-155、趋化因子配体17(CCL17)对小儿肺炎支原体(MP)感染的诊断价值及其与疾病严重程度的关系。方法选取乐清市人民医院2020年4月-2023年4月收治的71例小儿MP感染(研究组)与72名同期健康体检儿童(对照组),根据病情程度将研究组患儿分为轻症组(46例)、重症组(25例);比较研究组与对照组血清miR-223、miR-155、CCL17水平,比较轻症组与重症组患儿血清miR-223、miR-155、CCL17水平,分析血清miR-223、miR-155、CCL17与疾病严重程度相关性,分析miR-223、miR-155、CCL17对小儿MP感染的诊断价值。结果研究组血清miR-223、miR-155、CCL17水平较对照组高,且重症组较轻症组高(P<0.05);血清miR-223、miR-155、CCL17水平与疾病严重程度呈正相关,三者联合检测对小儿MP感染的诊断曲线下面积(AUC)值高于单独检测(P<0.05),且联合检测的敏感度为88.73%,特异度为88.89%。结论小儿MP感染后,血清miR-223、miR-155、CCL17水平升高,小儿MP感染重症患儿趋势变化更为明显,血清miR-223、miR-155、CCL17水平与病情严重程度密切相关,三者联合检测对小儿MP感染具有较高的诊断价值。OBJECTIVE To investigate the diagnostic value of serum micro ribonucleic acid(miR)-223,miR-155 and chemokine ligand 17(CCL17)in Mycoplasma pneumoniae(MP)infection in children and their relationship with the severity of the disease.METHODS A total of 71 cases of MP infection in children admitted to People's Hospital of Yueqing from Apr 2020 to Apr 2023(as the research group)and 72 healthy children under physical examination during the same period(as the control group)were recruited.The children in the research group were divided into the mild group(46 cases)and the severe group(25 cases)according to the severity of the disease.The serum levels of miR-223,miR-155 and CCL17 between the research group and the control group were compared,and the serum levels of miR-223,miR-155 and CCL17 between the mild and the severe groups were compared.The correlation between serum miR-223,miR-155 and CCL17 levels and the severity of the disease was analyzed.The diagnostic value of miR-223,miR-155 and CCL17 levels in the MP infection in children was analyzed.RESULTS Serum levels of miR-223,miR-155 and CCL17 in the research group were higher than those in the control group,and those in the severe group were higher than those in the mild group(P<0.05).Serum levels miR-223,miR-155 and CCL17 were positively correlated with disease severity,and the area under the curve(AUC)of the combined detection of the three indexes for the diagnosis of MP infection in children was larger than that of single detection(P<0.05),with the sensitivity and specificity of 88.73%and 88.89%,respectively.CONCLUSION After MP infection in children,the serum levels of miR-223,miR-155 and CCL17 increase,which is more obvious in children with severe infecting children with MP and closely related to the severity of the disease.The combined detection of serum miR-223,miR-155 and CCL17 levels have high diagnostic value for MP infection inchildren.
