睡眠质量在中老年糖尿病患者衰弱与抑郁间的中介效应

摘　　要：目的探讨睡眠质量在中老年糖尿病患者衰弱与抑郁间的中介效应。方法运用2020年北京大学《中国健康与养老追踪调查(CHARLS)》第五次全国追访数据,将880例年龄≥45岁的中老年糖尿病患者作为研究对象,采用SPSS 27.0进行描述性分析和Spearman相关分析,使用PROCESS宏程序进行中介效应检验。结果中老年糖尿病患者抑郁得分为7.5(3.0,14.0)分,衰弱指数为22.7(17.3,28.0)。女性、高龄、农村居住、现在吸烟、过去1年每个月饮酒≥1次及有跌倒经历的中老年糖尿病患者抑郁得分更高(P<0.05)。抑郁与衰弱呈正相关(r=0.757,P<0.01),与睡眠质量呈负相关(r=-0.722,P<0.01)。衰弱与睡眠质量呈负相关(r=-0.570,P<0.01)。衰弱可直接影响中老年糖尿病患者抑郁(β=0.562,t=30.143,P<0.001),还可通过睡眠质量的中介作用间接影响抑郁,间接效应为总效应的28.558%。结论睡眠质量是中老年糖尿病患者衰弱与抑郁之间的中介变量,应加强中老年糖尿病患者睡眠质量问题的预防和综合干预。Objective To explore the mediating effects of sleep quality between frailty and depression in middle-aged and elderly diabetic patients.Methods The data from the fifth national follow-up of the China Health and Retirement Longitudinal Study conducted by Peking University in 2020 were used,880 middle-aged and elderly diabetic patients aged≥45 years were selected.Descriptive and Spearman correlation analysis were performed using SPSS 27.0,and the mediating effect was tested using the PROCESS macro.Results The depression score of middle-aged and elderly diabetic patients was 7.5(3.0,14.0)points,and the frailty index was 22.7(17.3,28.0).Female,older age,rural residence,current smoking,drinking alcohol at least once a month in the past year,and having a history of falls were associated with higher depression scores(P<0.05).Depression was positively correlated with frailty(r=0.757,P<0.01),and negatively correlated with sleep quality(r=-0.722,P<0.01).Frailty was negatively correlated with sleep quality(r=-0.570,P<0.01).Frailty directly affected depression in middle-aged and elderly diabetic patients(β=0.562,t=30.143,P<0.001),and also indirectly affected depression through the mediating role of sleep quality,with the indirect effect accounting for 28.558%of the total effect.Conclusion Sleep quality is a mediating variable between frailty and depression in middle-aged and elderly diabetic patients.It is necessary to strengthen the prevention and comprehensive intervention of sleep quality issues in this population.
