摘　　要：目的:探讨益艾康胶囊对人类免疫缺陷病毒(human immunodeficiency virus,HIV)/获得性免疫缺陷综合征(acquired immunodeficiency syndrome,AIDS)肺脾气虚证患者CD38、HLA-DR分子表达及Th17/Treg平衡的影响。方法:纳入研究对象38例,其中包括HIV/AIDS肺脾气虚证患者18例(肺脾气虚组)及同地区健康人20例(健康对照组)。肺脾气虚组患者在口服抗病毒药的基础上服用中药制剂益艾康胶囊,干预6个月,每次5粒,每日3次。健康对照组不予干预。干预结束后,应用流式细胞仪对健康对照组和HIV/AIDS肺脾气虚组患者的T淋巴细胞亚群及CD38、HLA-DR、Th17、Treg细胞水平进行检测。结果:肺脾气虚组治疗后CD38^(+)CD8^(+)百分比较治疗前下降,CD38^(+)CD4^(+)百分比和Treg细胞水平较治疗前显著降低(P<0.05),HLA-DR^(+)CD4^(+)百分比、HLA-DR^(+)CD8^(+)百分比、Th17、Th17/Treg水平较治疗前显著升高(P<0.05)。结论:益艾康胶囊显著降低HIV/AIDS肺脾气虚证患者Treg细胞水平,升高Th17水平,调节Th17/Treg平衡及CD38、HLA-DR分子的异常表达。Objective:To investigate the effects of Yi′aikang Capsule on expression of CD38 and HLA-DR molecules and the balance of Th17/Treg in patients with human immunodeficiency virus(HIV)/acquired immunodeficiency syndrome(AIDS),characterized by syndrome of Lung and Spleen Qi deficiency.Methods:A total of 38 subjects were enrolled,including 18 HIV/AIDS patients with syndrome of Lung and Spleen Qi deficiency(syndrome of Lung and Spleen Qi deficiency group)and 20 healthy people in the same area(healthy control group).The patients in the syndrome of Lung and Spleen Qi deficiency group were given Yi′aikang Capsule on the basis of oral antiviral drugs for 6 months,5 capsules each time,3 times a day.The healthy control group received no intervention.After the intervention,the levels of T lymphocyte subsets,CD38,HLA-DR,Th17 and Treg cells of healthy control group and HIV/AIDS patients with syndrome of Lung and Spleen Qi deficiency were detected by flow cytometry.Results:In the syndrome of Lung and Spleen Qi deficiency group,the percentage of CD38^(+)CD8^(+)decreased after treatment,the percentage of CD38^(+)CD4^(+)and the level of Treg cells were significantly lower than those before treatment(P<0.05).The percentages of HLA-DR^(+)CD4^(+),HLA-DR^(+)CD8^(+),Th17 and Th17/Treg were significantly higher than those before treatment(P<0.05).Conclusion:Yi′aikang Capsule can significantly reduce the level of Treg cells,increase the level of Th17 cells,and regulate the balance of Th17/Treg as well as abnormal expression of CD38 and HLA-DR molecules in HIV/AIDS patients with syndrome of Lung and Spleen Qi deficiency.
