中医药辨治癌因性疲乏的临床用药规律

摘　　要：目的:基于隐含狄利克雷分布(Latent Dirichlet Allocation,LDA)主题模型等数据挖掘方法探讨中医药辨治癌因性疲乏(cancer-related fatigue,CRF)的临床用药规律。方法:计算机检索收集2000年至2022年在中国知网期刊全文数据库、万方学术期刊全文数据库、维普中文科技期刊数据库中主题为“中医药”“癌因性疲乏”“虚劳”的临床文献,采用频数统计、关联规则Apriori算法及LDA主题模型挖掘方法,分析高频药物的功效种类、性味归经、药对组合、处方结构。结果:研究共纳入62首方剂,包括142味中药,累计频次共630次。临床用药以补虚药为主,常用药物为黄芪、白术、甘草等。中药性味以温、甘居多,归经以脾、肺、胃经为主。支持度最高的药对为白术-甘草,同时挖掘出2首新方。结论:健脾和胃、气血双补是CRF的治疗大法,可为后续相关临床研究和新药开发提供参考。Objective:To explore the clinical medication rules of traditional Chinese medicine(TCM)in treating cancer-related fatigue(CRF)based on Latent Dirichlet Allocation(LDA)topic model and other data mining techniques.Methods:The clinical literatures with the topics of"traditional Chinese medicine","cancer-related fatigue"and"Xu Lao"in CNKI Journal Full-text Database,Wanfang Academic Journal Full-text Database as well as VIP Chinese Science and Technology Journal Database from 2000 to 2022 were searched and collected.Frequency statistics,association rule Apriori algorithm and LDA topic model mining methods were used to analyze the efficacy types,properties,tastes,drug pairs and prescription structure of high-frequency medicines.Results:A total of 62 prescriptions were included,with 142 Chinese medicinal kinds being used for a total of 630 times.The herbs commonly used are Huangqi(Astragali Radix),Baizhu(Atractylodis Macrocephalae Rhizoma),Gancao(Glycyrrhizae Radix et Rhizoma),etc.for mainly supplementing deficiency,which are mainly warm in property,sweet in taste,and the channel tropism cases are mainly the Spleen,Lung and Stomach meridians.The drug pairs with the highest support are Atractylodes Macrocephala and Glycyrrhiza.In addition,two new prescriptions were found.Conclusion:Invigorating Spleen and Stomach as well as supplementing Qi and blood are the main methods for the treatment of CRF,which provide reference for related clinical research and new drug development in future.
