结核丸联合规范化方案治疗肺结核合并糖尿病的临床效果

摘　　要：目的 观察结核丸联合规范化方案治疗肺结核合并糖尿病的临床效果。方法 回顾性选取2021年9月—2023年1月湖南省胸科医院收治的肺结核合并糖尿病患者112例,依据治疗方法不同分为观察组(n=60)和对照组(n=52)。对照组以HRZE方案强化治疗,观察组在HRZE方案基础上加用结核丸治疗,2组均治疗2个月。比较2组疗效,治疗前后中医证候积分、肺功能指标[第1秒用力呼气容积(FEV_(1))、1秒率(FEV_(1)/FVC)]、炎性因子[白介素-1受体1(IL-1R1)、γ-干扰素(IFN-γ)、C反应蛋白(CRP)]、免疫指标(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))。结果 观察组治疗总有效率为98.33%,高于对照组的86.54%(χ^(2)=4.200,P=0.040)。治疗2个月后,2组呛咳气急、五心烦热、颧红盗汗、口渴心烦证候积分,IL-1R1、IFN-γ、CRP水平,CD8^(+)低于治疗前;FVC、FEV_(1)/FVC,CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于治疗前,且观察组低/高于对照组(P<0.01)。结论 结核丸联合规范化方案治疗肺结核合并糖尿病效果良好,可减轻患者中医证候,改善患者肺功能,降低炎性因子水平,增强患者免疫功能。Objective To observe the clinical effect of tuberculosis pills combined with standardized program in the treatment of pulmonary tuberculosis complicated with diabetes.Methods A total of 112 patients with pulmonary tuberculosis combined with diabetes admitted to Hunan Chest Hospital from September 2021 to January 2023 were selected retrospectively,and divided into observation group(n=60) and control group(n=52) according to different treatment methods.The control group was treated with HRZE regimen,the observation group was treated with tuberculosis pills on the basis of HRZE regimen,and both groups were treated for two months.The efficacy,TCM syndrome score,lung function index[forced expiratory volume in the first second(FEV_1),one-second rate(FEV_1/FVC)],inflammatory factor[interleukin-1 receptor 1(IL-1R1),γ-interferon(IFN-γ),C-reactive protein(CRP)],immune index(CD3~+,CD4~+,CD8~+,CD4~+/CD8~+) before and after treatment were compared between two groups.Results The total effective rate of the observation group was 98.33%,which was higher than 86.54% of the control group(χ^(2)=4.200,P=0.040).After two months of treatment,the scores of choking and gasping,five upset fever,zygomatic red night sweats and thirst upset,the levels of IL-1R1,IFN-γ,CRP,and CD8~+ in two groups were lower than before treatment,FVC,FEV_1/FVC,and CD3~+,CD4~+,CD4~+/CD8~+ in two groups were higher than before treatment,and the observation group had lower/higher values for the above indicators than control group(P<0.01).Conclusion Tuberculosis pills combined with standardized program has good effect in the treatment of pulmonary tuberculosis complicated with diabetes,which can reduce the TCM syndrome of patients,improve the lung function of patients,reduce the level of inflammatory factors and enhance the immune function of patients.
