摘 要:目的探讨百日咳与类百日咳综合征患儿临床特征、肠道菌群及肠道黏膜屏障功能的差异。方法选取2020年1月至2023年12月于河北北方学院附属第一医院就诊的40例百日咳儿童为百日咳组,并同期收集43例类百日咳综合征患儿为类百日咳组。比较两组临床特征、肠道黏膜屏障功能及肠道菌群的差异;分析两组临床特征、肠道菌群与肠黏膜屏障功能的相关性。结果百日咳组百日咳疫苗接种占比低于类百日咳组;青紫占比、白细胞计数(WBC)、淋巴细胞百分比高于类百日咳组(P<0.05)。百日咳组肠道双歧杆菌、乳酸杆菌数量低于类百日咳组,大肠埃希菌数量高于类百日咳组(P<0.05)。百日咳组D-乳酸(D-LA)和二胺氧化酶(DAO)水平高于类百日咳组(P<0.05)。百日咳组中,青紫、WBC、淋巴细胞百分比和大肠埃希菌数量与D-LA和DAO水平呈正相关(rs/r>0,P<0.05);百日咳疫苗接种史及双歧杆菌、乳酸杆菌数量与D-LA和DAO水平呈负相关(rs/r<0,P<0.05)。类百日咳组中,青紫、WBC、淋巴细胞百分比和大肠埃希菌数量与D-LA和DAO水平呈正相关(rs/r>0,P<0.05);双歧杆菌、乳酸杆菌数量与D-LA和DAO水平呈负相关(rs/r<0,P<0.05)。结论与类百日咳综合征比较,百日咳患儿接种百日咳疫苗的比例较低,WBC和淋巴细胞百分比较高,存在肠道菌群失衡,肠道黏膜屏障受损。Objective To explore the differences in clinical characteristics, intestinal flora, and intestinal mucosal barrier function in children with pertussis and pertussis-like syndrome. Methods Forty cases of children diagnosed with pertussis who had attended the First Affiliated Hospital of Hebei North University from January 2020 to December 2023 were selected as the pertussis group, while 43 cases of children with pertussis-like syndrome during the same period were selected as the pertussis-like group. The differences in clinical characteristics, intestinal mucosal barrier function, and intestinal flora between the two groups were compared;and the correlation between clinical characteristics, intestinal flora, and intestinal mucosal barrier function were analyzed. Results The percentage of pertussis vaccination in the pertussis group was lower than that in the pertussis-like group, while the percentage of cyanosis, white blood cell count (WBC), and lymphocyte percentages were higher than those in the pertussis-like group (P<0.05). Intestinal Bifidobacterium and Lactobacillus counts in the pertussis group were lower than those in the pertussis-like group, while Escherichia coli counts were higher than those in the pertussis-like group (P<0.05). The levels of D-lactic acid (D-LA) and diamine oxidase (DAO) in the pertussis group were higher than those in pertussis-like syndrome group (P<0.05). In the pertussis group, cyanosis, WBC, lymphocyte percentage, and Escherichia coli counts were positively correlated with the levels of D-LA and DAO (rs/r>0, P<0.05);pertussis vaccination history, Bifidobacterium and Lactobacillus counts were negatively correlated with the levels of D-LA and DAO (rs/r<0, P<0.05). In pertussis-like group, cyanosis, WBC, lymphocyte percentage, and Escherichia coli counts were positively correlated with the levels D-LA and DAO (rs/r>0, P<0.05);Bifidobacterium and Lactobacillus counts were negatively correlated with the levels of D-LA and DAO (rs/r<0, P<0.05).
