摘　　要：本文基于“寒胜则浮”中对“浮”的解释,即形体浮肿、神气外浮,从形神两个方面出发,阐述寒邪致痛的病机特点及临床治疗。从寒胜形浮来看,可分为寒伤形体及阳虚寒化两个部分,主以形体浮肿,不通则痛,不荣则痛;从寒胜神浮来看,可分为心神内动及阳虚敏化两个部分,主以神气外浮,痛阈下降,感邪即痛。提出散寒以消形浮、温阳以安神浮是痛证治疗的关键。从“寒胜则浮”理论探析痛证,旨在进一步阐明其发病机制,为现代临床从寒论治疼痛性疾病提供新的理论依据及思路。Based on the explanation of“edema”in“cold domination inducing edema”,that is body edema and spirit flotation,this paper expounds the characteristics of pathogenesis and clinical treatment of pain caused by cold evil from the aspects of body and spirit.From the perspective of cold injury to the body,it can be divided into two parts:cold injury to the body and cold transformation of yang deficiency,mainly by body edema and pain caused by obstruction or malnutrition.From the perspective of cold injury to the spirit,it can be divided into two parts:spirit perception and sensitization of yang deficiency,mainly by spirit floating and pain threshold declining and pain appearing.It is suggested that dispelling cold and channeling pulse,warming yang and regulating spirit are the key points of pain syndrome treatment.Theoretical discussion of pain syndrome based on“cold domination inducing edema”aims to further clarify its pathogenesis and to provide new theoretical basis and ideas for modern clinical treatment of pain diseases.
