摘　　要：耐药性癫痫是指癫痫患者在接受≥2种抗癫痫药物治疗后仍无法控制的一种复杂发作性疾病,属于中医学“痫证”的范畴。刘金民教授认为少阳转枢不利是耐药性癫痫的核心病机,病因涉及气郁、痰瘀、精血亏虚等多种因素,治疗上倡导从“和少阳,调枢机”角度入手,将“开阖枢机、通利三焦、补虚培元”法贯穿遣方用药的全程,善用柴胡剂,喜用风药、虫药和补药,临床疗效显著。本文总结刘金民教授治疗耐药性癫痫的理论思想与辨治思路,并附1则验案加以阐释,以期为该病的防治提供新的视角和实践指导。Drug-resistant epilepsy refers to a complex episodic disease that epilepsy patients cannot control even after receiving treatment with more than two antiepileptic drugs,and belongs to the category of“epilepsy syndrome”in traditional Chinese medicine.Professor Liu Jinmin believes that the unfavorable pivot of shaoyang is the core pathogenesis of drug-resistant epilepsy,which involves various factors such as qi stagnation,phlegm stasis,and deficiency of essence and blood.In terms of treatment,it is advocated to start from the perspective of“harmonizing shaoyang and regulating the pivoto”,and to integrate the methods of“opening and closing the pivot,unblocking the triple burner,and tonifying deficiency and supporting the healthy energy”throughout the entire process of prescription and medication.Chaihu Decoction is well used,and wind medicine,insect medicine,and tonics are preferred,resulting in significant clinical efficacy.This article summarizes Professor Liu’s theoretical ideas and treatment strategies for drug-resistant epilepsy,and provides one case study for explanation,in order to provide new perspectives and practical guidance for the prevention and treatment of this disease.
