渐进式引导护理法结合重返社区技能训练对精神分裂症患者疾病症状和风险行为的影响

摘　　要：目的探究渐进式引导护理法结合重返社区技能训练对精神分裂症患者疾病症状和风险行为的影响。方法选取2020年4月至2023年4月于南京医科大学附属脑科医院诊断为精神分裂症并住院治疗的102例患者为研究对象,采用随机数字表法将其分为对照组和观察组,各51例。对照组实施常规护理+渐进式引导护理法,观察组在对照组基础上采用重返社区技能训练。比较两组护理干预前及干预后3个月的简明精神病评定量表(BPRS)、修订版外显攻击行为量表(MOAS)、一般疏离感量表(GAS)、阳性与阴性症状量表(PANSS)评分及护理期间高风险行为发生情况。结果干预前,两组BPRS、MOAS、GAS及PANSS评分比较,差异无统计学意义(P>0.05)。干预后3个月,两组BPRS、MOAS、GAS及PANSS评分均低于干预前,且观察组低于对照组(P<0.05)。护理期间研究组高风险行为总发生率为10.42%,明显低于对照组的27.45%(P<0.05)。结论渐进式引导护理法结合重返社区技能训练能有效降低精神分裂症患者的疾病症状和风险行为,显著提高生活质量,值得在临床上进一步推广应用。Objective To explore the effects of progressive guidance nursing combined with reintegration into community skills training on the symptoms and risk behaviors of patients with schizophrenia.Methods From April 2020 to April 2023,102 patients diagnosed with schizophrenia and received inpatient treatment at the Affiliated Brain Hospital of Nanjing Medical University were assigned into control group and observation group by random number table method,each with 51 patients.The control group received routine care plus progressive guidance nursing,whereas the observation group additionally received reintegration into community skills training.The brief psychiatric rating scale(BPRS),the modified overt aggression scale(MOAS),the general alienation scale(GAS),the positive and negative syndrome scale(PANSS),and the incidence of high-risk behaviors during the nursing were compared before and three months after the intervention.Results Before intervention,there were no significant differences in the BPRS,MOAS,GAS,and PANSS scores between the two groups(P>0.05).Three months post-intervention,scores of BPRS,MOAS,GAS,and PANSS in both groups were lower than pre-intervention,those of the observation group were lower than the control group(P<0.05).The total incidence of high-risk behaviors in the observation group was 10.42%,was lower than 27.45%in the control group(P<0.05).Conclusion Progressive guidance nursing combined with community reintegration into community skills training effectively reduces symptoms and risk behaviors in patients with schizophrenia,significantly enhancing the quality of life.This approach is worthy of further clinical promotion and application.
