消疣汤治疗HR-HPV感染相关LSIL的临床观察及其对阴道微生态的影响

摘　　要：目的观察消疣汤治疗湿热下注型高危型人乳头瘤病毒(HR-HPV)感染相关低级别鳞状上皮内病变(LSIL)的临床疗效和对阴道微生态(VM)的影响。方法将2022年1月至2022年12月收集的90例受试者随机分为试验组和对照组各45例。试验组给予消疣汤阴道灌洗,对照组给予重组人干扰素α-2b凝胶阴道纳药,治疗周期3个月。停药30天后对比两组患者HR-HPV转阴率、TCT恢复情况、中医证候积分及VM的变化,记录用药期间不良安全性事件。结果试验组HR-HPV转阴率为64.29%,高于对照组的40.00%(P<0.05);试验组TCT恢复率为71.43%,高于对照组的42.50%(P<0.05);两组治疗前后中医证候积分均显著下降(P<0.01),试验组显著优于对照组(P<0.01);治疗后试验组VM恢复率为30.95%,高于对照组的10.00%(P<0.05),其中菌群多样性、白细胞、过氧化氢、pH、白细胞酯酶异常检出率较对照组低(P<0.05);两组均未出现不良安全性事件。结论消疣汤治疗湿热下注型HR-HPV感染相关LSIL的患者在清除HR-HPV、使TCT恢复正常、改善中医临床症状、恢复VM方面疗效显著。Objective To observe the clinical effect of Xiaoyou decoction in treating damp-heat type Human Papilloma Virus(HR-HPV)infection related Low-grade Squamous Intraepithelial Lesion(LSIL)and its influence on Vaginal Microecology(VM).Methods 90 patients from January 2022 to December 2022 were randomly divided into experimental group and control group,with 45 patients in each group.The experimental group was given vaginal lavage with Xiaoyou decoction,and the control group was applied recombinant human interferonα-2b gel to vagina.The treatment period was 3 months.After 30 days,observe the HR-HPV negative conversion rate,the recovery of TCT,TCM syndrome scores and VM changes were compared between the two groups,and adverse safety events were recorded.Results The HR-HPV negative conversion rate was 64.29%,which was higher than the control group at 40.00%(P<0.05).The HR-HPV negative conversion rate was 64.29%,which was higher than the control group at 40.00%(P<0.05),and the TCT recovery rate was 71.43%,which was higher than the control group at 42.50%(P<0.05).The TCM syndrome scores of both groups decreased significantly after treatment(P<0.01).The VM recovery rate of the experimental group was 30.95%,which was significantly higher than the control group at 10%(P<0.05).Moreover,the diversity of flora,leukocytes,hydrogen peroxide,pH and leukocyte esterase in the experimental group after treatment were lower than those in the control group(P<0.05).Conclusion Xiaoyou Decoction has significant efficacy in the treatment of damp-heat type HR-HPV infection related LSIL at clearing HR-HPV,restoring TCT to normal,improving clinical symptoms of traditional Chinese medicine,and restoring vaginal microecology.
