临床药师参与1例食管癌患者奥默毕赤酵母菌血流感染的治疗分析

摘　　要：临床药师参与1例食管癌患者奥默毕赤酵母菌血流感染的诊疗工作。通过查阅相关病例报道并总结既往奥默毕赤酵母菌感染患者的临床特征及治疗药物,结合该患者的基础疾病、临床症状和实验室检查,为患者提供抗感染治疗方案及药学监护。该患者外周血和导管血培养均检出奥默毕赤酵母菌,临床药师先后建议给予伏立康唑和两性霉素B治疗。经两性霉素B治疗后,患者病情较前明显好转,血培养转阴,为后续抗肿瘤治疗奠定了基础。两性霉素B可能是奥默毕赤酵母菌所致导管相关血流感染的优选药物之一。临床药师积极参与罕见真菌血流感染的治疗,能够提高患者的治疗获益。Clinical pharmacists participated in the treatment of blood flow infection of Pichia ohmeri in a patient of esophageal cancer.In light of underlying diseases,clinical symptoms and laboratory tests,antibiotics and pharmaceutical care were provided.Pichia ohmeri was detected in peripheral and catheter blood samples.Voriconazole and amphotericin B were recommended by clinical pharmacists.After amphotericin B dosing,patient status improved markedly and the result of blood culture turned nega⁃tive,laying the foundations for subsequent anti-neoplastic treatment.Amphotericin B is an optimal drug for catheter-associated bloodstream infections caused by Pichia ohmeri.Clinical pharmacists actively participating in the treatment of rare fungal blood⁃stream infection may enhance the treatment benefits of patients.
