摘 要:目的:探讨外周血中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)与小儿百日咳肺炎的关系及其临床意义。方法:选取2021年8月-2022年7月在该院确诊的59例百日咳肺炎以及99例非百日咳肺炎患儿作为研究对象,记录住院患者初次血常规外周血中性粒细胞计数、淋巴细胞计数以及血小板计数,并计算NLR、PLR,用t检验比较NLR、PLR在百日咳组和对照组之间的差异,用受试者工作特征曲线(ROC曲线)确定NLR、PLR在百日咳肺炎中鉴别的临界值。结果:百日咳组全血中淋巴细胞计数、血小板计数、PLR高于对照组;C-反应蛋白低于对照组(P<0.05)。各项指标在诊断百日咳肺炎的受试者工作特征曲线中淋巴细胞、PLR、NLR、CRP的最佳截断值分别为4.02、6704.29、181.16、1.89,诊断的灵敏度中以PLR最高,为82.8%,特异度中以NLR最高为86.4%。结论:NLR、PLR及四项指标联合检测在百日咳肺炎与非百日咳肺炎鉴别中具有重要的参考意义。Objective:To investigate the relationship between peripheral blood neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)and pertussis pneumonia in children and their clinical significance.Methods:A total of 59 cases of pertussis pneumonia and 99 cases of non-pertussis pneumonia diagnosed in the hospital from August 2021 to July 2022 were selected as research objects.The neutrophils count,lymphocyte count and platelet count in peripheral blood were recorded during the initial routine blood transfusion.The difference ofNLR and PLR between the pertussis group and the control group was compared by t test.The critical value of NLR and PLR in pertussis pneumonia was determined by Receiver Operating Characteristic curve(ROC curve).Results:The lymphocytes and platelets in whole blood of pertussis group and PLR were higher than those in control group.C-reactive protein were lower than those in control group(P<0.05).The optimal cut-offvalues oflymphoid cells,PLR,NLR and CRP were 4.02,6704.29,181.16 and 1.89,respectively.The sensitivity of diagnosis was the highest in PLR(82.8%),and the specificity was the highest in NLR(86.4%).Conclusion:The combined determination of NLR,PLR and the four indexes has important reference significance in the differentiation of pertussis pneumonia and non-pertussis pneumonia.
