基于“玄府-气液-络脉”理论探析糖尿病胃轻瘫病机与治法

摘　　要：糖尿病胃轻瘫属于气血津液类疾病,脾胃位于中焦,主运化,为气血津液生成、输布之枢纽。玄府与络脉为气液升降出入、运行周身的微观结构,气液为气血津液等精微物质的统称。糖尿病胃轻瘫发病、发展及恶化与玄府、气液、络脉三者的病变关系密切。糖尿病胃轻瘫常因脾胃玄府郁闭,精微不散,蓄于脉中使血糖升高而发病,随着病情发展,气液失宣,络脉瘀阻,加剧病情。故临床治疗以芳香开玄、调中畅液、辛润通补为治则,随证加减。本文旨在从“玄府-气液-络脉”角度探讨糖尿病胃轻瘫的病机演变规律和治疗方法,以期为中医临床治疗糖尿病胃轻瘫提供参考。Diabetic gastroparesis is a type of disease related to qi,blood and body fluid.The spleen and stomach are located in the middle energizer,which is the hub for the generation and distribution of qi,blood and liquid.Sweat pore and collateral are the microscopic structures of qi and liquid ascending and descending,entering and exiting,and running throughout the whole body,while qi and liquid are the general name of fine substances such as qi,blood and body fluid.So the onset,development and deterioration of diabetes gastroparesis are closely related to the pathological changes of sweat pore,qi and liquid,and collaterals.Diabetes often occurs when the sweat pores of the spleen and stomach are closed,essence cannot be dispersed and accumulated in the vessels,which causes blood sugar to rise.Along with the development of the disease,the patient’s body fails to diffuse qi and liquid,further aggravating the condition.Therefore,clinical treatment of the disease should follow the principles of“opening the orifices with fragrance,adjusting the middle to promote the flow of liquids,using pungent and moistening medicinals to unblock and supplement the body.”The purpose of this paper is to explore the pathogenesis evolution rules and treatment methods of diabetic gastroparesis from the perspective of“sweat pore-qi and fluid-collaterals”,so as to provide reference for the clinical treatment of diabetes gastroparesis in TCM.
