基于形气神一体论浅探重症肌无力辨治思路

摘　　要：重症肌无力是一种罕见性的神经系统疾病,以肌无力和易疲劳为主要临床表现,其精神症状常被忽视,西医治疗不良反应明显且易复发。形气神一体论作为中医学认知生命体的独特思维模式,贯穿重症肌无力发生发展之始终。故本文基于形气神一体论阐释重症肌无力的病理机制及诊疗思路,认为其病机涉及精血亏耗、脏热脉阻、邪滞形弱所致之形痿,气虚阳衰、阳明阖折、营卫失调所生之气陨,神失宅所、神气乏源、神不导气所见之神郁。治疗则以实形、益气、调神为要,最终达到恢复形气神自稳态的目的,以期为重症肌无力的临床治疗提供中医药视角和方案。Myasthenia gravis(MG)is a rare neurological disease with myasthenia and fatigue as its main clinical manifestations,with psychological symptoms often being overlooked.Western medicine treatments for MG have noticeable side effects and a high risk of relapse.The theory of the integration of form,qi,and spirit in traditional Chinese medicine(TCM)provides a unique perspective on understanding the pathogenesis and treatment of MG.This article aims to explore the pathological mechanisms and diagnostic strategies of MG based on the theory of the integration of form,qi,and spirit.It suggests that the pathogenesis of MG involves form flaccidity caused by essence and blood depletion,visceral heat and meridian obstruction,and pathogenic stagnation and physical weakness;qi decline caused by deficiency of yang qi,obstruction of the yangming meridian,and disharmony of nourishing and protecting qi;and spirit stagnation characterized by loss of residence,lack of spirit essence,and impaired spirit guidance.The treatment approach mainly focuses on tonifying qi,regulating the form and spirit,aiming to restore the self-stabilizing state of form,qi,and spirit.This article provides a TCM perspective and treatment plan for the clinical management of MG.
