摘 要:目的:探讨多排螺旋CT联合数字X线平片(DR)检查在腰椎间盘突出症诊断中的应用价值。方法:抽取2021年10月至2023年10月安康市人民医院收治的110例腰椎间盘突出症患者的临床资料进行回顾性分析。入组患者均接受多排螺旋CT与DR检查,以手术结果为金标准,对患者的检查结果进行分析。结果:DR检查显示腰椎间盘突出症患者腰椎出现正常生理弯曲变化,部分患者侧弯为畸形,呈后弓状;腰椎间隙前后左右四个方向的宽度出现差异,即病变腰椎间隙也出现不等宽,凹侧窄、凸侧宽,前方窄、后方宽,腰椎体上或下方有凹弧形压迹。手术检查结果显示腰椎间盘突出症分型占比前两位,分别为中央型(36.36%,40/110)和椎间孔型(35.45%,39/110),其次旁侧型(14.55%,16/110)和极外侧型(13.64%,15/110);CT诊断极外侧型、椎间孔型、旁侧型、中央型的符合率分别为78.95%(15/19)、92.31%(36/39)、81.25%(13/16)、95.24%(40/42);腰椎间盘突出病灶位于第3~4腰椎37例,第4~5腰椎59例,第5腰椎~第1骶椎14例。联合诊断的符合率高于DR与多排螺旋CT单项检查(P<0.05)。结论:多排螺旋CT联合DR检查能清晰呈现腰椎间盘突出症患者腰椎病变情况,辅助临床诊断,特别是对于中青年专科医生,能为评估腰椎间盘突出症患者病情提供重要参考依据。Objective:To investigate the application value of multi-section helical computed tomography(CT)combined with X-ray plain film(DR)examination in the diagnosis of lumbar disc herniation.Methods:Retrospective analysis was conducted on the clinical data of 110 patients with lumbar disc herniation admitted to Ankang People’s Hospital from October 2021 to October 2023.All enrolled patients underwent multi-section helical CT and DR examinations.The surgical results were used as the gold standard to analyze the examination results of the patients.Results:According to DR examination,patients with lumbar disc herniation had normal changes in physiological curvature of the lumbar spine,and some patients had lateral curvature deformities,manifesting a posterior arch shape.The width of the lumbar intervertebral space in the four directions of front,back,left,and right varied.It indicated that the diseased lumbar intervertebral space had unequal width,with a narrow concave side and a wider convex side,a narrow front and a wide back,and a concave arc impression on the upper or lower part of the lumbar vertebral body.The results of the surgery showed that the top two types of lumbar disc herniation were central type(36.36%,40/110)and intervertebral foramen type(35.45%,39/110),followed by lateral type(14.55%,16/110)and extreme lateral type(13.64%,15/110).The coincidence rate of CT in the diagnosis of the extreme lateral,intervertebral foramen,lateral,and central types were 78.95%(15/19),92.31%(36/39),81.25%(13/16),and 95.24%(40/42),respectively;the lesion of lumbar disc herniation located at the 3rd to 4th lumbar vertebrae in 37 cases,at the 4th to 5th lumbar vertebrae in 59 cases,and at the 5th lumbar vertebra to the 1st sacral vertebra in 14 cases.The coincidence rate of combined diagnosis was higher than that of DR alone and multi-section helical CT alone(P<0.05).Conclusions:Multi-section helical CT combined with DR can clearly delineate the lesion condition of lumbar disc herniation,and can assist clinical diagnosis,especia
