黄连生地汤联合艾司唑仑片治疗阴虚火旺型失眠临床研究

摘　　要：目的:观察黄连生地汤联合艾司唑仑片治疗阴虚火旺型失眠的临床疗效。方法:将104例阴虚火旺型失眠患者按随机数字表法分为对照组和观察组各52例。对照组采用艾司唑仑片治疗,观察组在对照组基础上联合黄连生地汤治疗,治疗1个月。评价2组临床疗效,比较2组治疗前后中医证候评分、睡眠质量、觉醒程度、血清5-羟色胺(5-HT)、多巴胺(DA)水平,统计不良反应发生率。结果:观察组总有效率为96.15%,高于对照组82.69%(P<0.05)。治疗后,2组各项中医证候评分较治疗前下降(P<0.05),且观察组各项中医证候评分低于对照组(P<0.05)。治疗后,2组匹兹堡睡眠质量指数量表(PSQI)、过度觉醒量表(HAS)评分较治疗前降低(P<0.05),且观察组PSQI、HAS评分低于对照组(P<0.05)。治疗后,2组5-HT水平较治疗前上升,DA水平较治疗前下降(P<0.05);且观察组5-HT水平高于对照组,DA水平低于对照组(P<0.05)。观察组不良反应发生率为3.85%,低于对照组15.38%(P<0.05)。结论:黄连生地汤联合艾司唑仑片治疗阴虚火旺型失眠患者疗效确切,能够改善患者失眠症状,减少觉醒次数,提高睡眠质量,安全性高,其作用机制可能和提高5-HT水平,降低DA水平有关。Objective:To observe the clinical effect of Huanglian Shengdi Decoction combined with Estazolam Tablets on insomnia of yin deficiency and vigorous fire type.Methods:A total of 104 cases of patients with insomnia of yin deficiency and vigorous fire type were divided into the control group and the observation group according to the random number table method,with 52 cases in each group.The control group was treated with Estazolam Tablets,and the observation group was additionally treated with Huanglian Shengdi Decoction based on the treatment of the control group.Both groups were treated for one month.Clinical effects were evaluated in the two groups.Traditional Chinese medicine syndrome scores,sleep quality,awakening degree,and the levels of serum 5-hydroxytryptamine(5-HT)and dopamine(DA)were compared between the two groups before and after treatment.The incidence of adverse reactions was counted.Results:The total effective rate was 96.15%in the observation group,higher than that of 82.69%in the control group(P<0.05).After treatment,each traditional Chinese medicine syndrome score in the two groups was decreased when compared with that before treatment(P<0.05),and each score in the observation group was lower than that in the control group(P<0.05).After treatment,the scores of Pittsburgh Sleep Quality Index(PSQI)and Hyperarousal Scale(HAS)in the two groups were reduced when compared with those before treatment(P<0.05),and the scores of PSQI and HAS in the observation group were lower than those in the control group(P<0.05).After treatment,5-HT levels in the two groups were increased when compared with those before treatment(P<0.05),and DA levels were decreased when compared with those before treatment(P<0.05);5-HT level in the observation group was higher than that in the control group,and DA level was lower than that in the control group(P<0.05).The incidence of adverse reactions was 3.85%in the observation group,lower than that of 15.38%in the control group(P<0.05).Conclusion:Huanglian Shengdi Decoction combined
