中药药枕辅助治疗维持性血液透析患者失眠临床研究

摘　　要：目的:观察中药药枕辅助治疗维持性血液透析患者失眠的临床疗效。方法:选取70例行维持性血液透析的失眠患者,按随机数字表法分为治疗组和对照组各35例。2组均予以肾脏一体化及规律血液透析治疗,治疗组加中药药枕辅助治疗,对照组加安慰枕治疗,2组疗程均为3个月。比较2组临床疗效及治疗前后匹兹堡睡眠质量指数(PSQI)、汉密尔顿焦虑量表(HAMA)评分、中医证候积分。结果:治疗后,治疗组总有效率91.43%,高于对照组62.86%(P<0.05)。治疗后,2组HAMA评分、中医证候积分、PSQI维度(睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、日间功能障碍)评分及总分均低于治疗前(P<0.05),且治疗组HAMA评分、中医证候积分、PSQI评分低于对照组(P<0.05)。结论:中药药枕辅助治疗维持性血液透析患者失眠疗效显著,能够有效改善患者的睡眠及生活质量。Objective:To observe the clinical effect of Chinese medicine pillow in the treatment of insomnia in maintenance hemodialysis patients.Methods:A total of 70 insomnia patients undergoing maintenance hemodialysis were selected and divided into the treatment group and the control group according to random number table method,with 35 cases in each group.Kidney integration and regular hemodialysis treatment were given to both groups.The treatment group was treated with Chinese medicine pillow and the control group was treated with comfort pillow.The course of treatment of both groups was three months.The clinical efficacy and scores of Pittsburgh Sleep Quality Index(PSQI)and Hamilton Anxiety Scale(HAMA)and Traditional Chinese medicine(TCM)syndrome scores before and after treatment were compared between the two groups.Results:After treatment,the total effective rate was 91.43%in the treatment group,higher than that of 62.86%in the control group(P<0.05).After treatment,HAMA score,TCM syndrome scores,and PSQI dimension(sleep quality,time to fall alseep,sleep duration,sleep efficiency,sleep disorder,daytime dysfunction)score and total score of the two groups were lower than before treatment,and HAMA score,TCM syndrome scores,and PSQI score of the treatment group were lower than that of the control group(P<0.05).Conclusion:Chinese medicine pillows adjuvant treatment can have a significant therapeutic effect on insomnia patients complicated with maintenance hemodialysis,which can effectively improve their sleep and quality of life.
