活血祛瘀汤对胫腓骨骨折髓内钉内固定术后炎症反应影响的临床研究

摘　　要：目的 探讨活血祛瘀汤对胫腓骨骨折髓内钉内固定术后患者的炎症反应影响及临床疗效.方法 将60例胫腓骨骨折髓内钉内固定术后患者随机分为治疗组和对照组,治疗组30例,对照组30例.对照组给予术后常规药物治疗,治疗组在对照组的基础上给予活血祛瘀汤治疗,连续治疗7d.治疗前后以血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平、患肢肿胀减少程度、肿胀消退时间、下肢运动功能评分(FMA评分)为观察指标.结果 治疗后,治疗组的血清CRP、TNF-α、IL-6水平均有所下降,均显著低于对照组(P<0.05).治疗组的患肢肿胀减少程度及肿胀消退时间均优于对照组,差异有统计学意义(P<0.05).两组患者FMA评分较治疗前相比均明显升高,且治疗组高于对照组(P<0.05).结论 活血祛瘀汤可以显著降低胫腓骨骨折髓内钉内固定术后患者的血清炎症因子,抑制炎症反应,改善下肢运动功能,值得推广.Ob jective To investigate the effect of Huoxue Quyu Decoction on the inflammatory response and clinical efficacy of patients after intramedullary nail fixation for tibiofibular fractures.Methods Sixty patients with tibiofibular fracture after intramedullary nail fixation were randomly divided into a treatment group and a control group,with 30 patients in each group.The control group received conventional postoperative drug treatment,while the treatment group received Huoxue Quyu Decoction in addition to the conventional treatment for 7 consecutive days.Serum C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)levels,reduction of limb swelling,swelling subsidence time,and lower extremity motor function score(FMA score)were observed before and after treatment.Results After treatment,the serum CRP,TNF-α,and IL-6 levels in the treatment group decreased significantly compared with the control group(P<0.05).The degree of reduction in limb swelling and the time of swelling subsidence in the treatment group were significantly beter than those in the control group(P<0.05).The FMA scores of both groups were significantly higher than those before treatment,and the treatment group was higher than the control group(P<0.05).Conclusion Huoxue Quyu Decoction can significantly reduce serum inflammatory factors,inhibit inflammatory reactions,and improve lower extremity motor function in patients after intramedullary nail fixation for tibiofibular fractures,which is worthy of promotion.
