化湿降浊饮治疗脾虚湿阻型老年痛风临床观察

摘　　要：目的观察化湿降浊饮治疗脾虚湿阻型老年痛风的临床效果,探讨患者中医证候与临床特点的关系。方法选取郑州市中医院肾病风湿科2020年9月—2021年6月门诊及住院处收治的老年痛风患者70例,随机分为两组,各35例。观察组给予化湿降浊饮口服,对照组给予非布司他片口服。比较两组治疗前后的尿酸水平、中医证候积分及疗效。结果观察组总有效率为82.86%(29/35),对照组为88.57%(31/35),差异无统计学意义(P>0.05)。治疗后,两组中医证候积分、血尿酸水平均低于治疗前(P<0.05)。老年痛风患者中医证候积分与血尿酸水平呈正相关(P<0.05)。结论化湿降浊饮治疗脾虚湿阻型老年痛风的效果显著。老年痛风患者的体质可能会引起血尿酸水平波动,从而导致痛风的发生发展。Objective To observe the clinical efficacy of Huashi Jiangzhuo drink in the treatment of elderly gout with dampness resis⁃tance of spleen deficiency type,and to explore the relationship between TCM syndrome and clinical features.Methods A total of 70 el⁃derly outpatients and inpatients with gout in Zhengzhou Hospital of Traditional Chinese Medicine from September 2020 to June 2021 were selected,and 70 subjects met the inclusion criteria were included.The patients were randomly divided into observation group of 35 cases and control group of 35 cases.The observation group was given oral Huashi Jiangzhuo drink,and the control group was given febuxostat tablets orally.The changes of serum uric acid and TCM syndrome score before and after treatment were compared between the two groups.Results The total effective rate was 82.86%(29/35)in the observation group and 88.57%(31/35)in the control group,and there was no statistical significance(P>0.05).After treatment,TCM syndrome score and blood uric acid level in both groups were lower than before treatment(P<0.05).There was a positive correlation between TCM syndrome score and blood uric acid level in el⁃derly gout patients(P<0.05).Conclusion Huashi Jiangzhuo drink has a significant effect on elderly gout with dampness resistance of spleen deficiency type.The constitution of elderly patients with gout may cause fluctuations in blood uric acid level,which leads to the occurrence and development of gout.
