补气活血汤配合热敏灸治疗椎动脉型颈椎病临床观察

摘　　要：目的研究补气活血汤配合热敏灸治疗椎动脉型颈椎病的效果。方法根据入选标准,选取新余市中医院2021年1月—2022年10月门诊及住院处收治的椎动脉型颈椎病患者60例,随机分为两组,每组30例,分别编入中医治疗组(补气活血汤配合热敏灸治疗)、西药对照组(盐酸氟桂利嗪治疗),统计分析两组患者治疗前后主要症状积分、颅脑彩色多普勒(TCD)检查结果及临床疗效。结果治疗后,中医治疗组患者的眩晕、头颈肩痛、耳鸣耳聋眼花、恶心呕吐、猝倒、健忘失眠积分均低于西药对照组(P<0.05),基底动脉、左椎动脉、右椎动脉血流速度及收缩期峰值血流速度、血管搏动指数均高于西药对照组(P<0.05),血管阻力指数低于西药对照组(P<0.05)。中医治疗组总有效率93.33%(28/30)高于西药对照组的70.00%(21/30)(P<0.05)。结论补气活血汤配合热敏灸治疗椎动脉型颈椎病的临床效果较西药好,能更有效地减轻患者症状,改善患者脑血流动力学指标。Objective To study the effect of Buqi Huoxue decoction combined with heat sensitive moxibustion in the treatment of cervi⁃cal spondylosis of vertebral artery type.Methods According to the inclusion criteria,60 cervical spondylosis of vertebral artery type outpatients and inpatients in Xinyu Hospital of Traditional Chinese Medicine from January 2021 to October 2022 were selected.All the medical records were randomly divided into two groups,with 30 cases in each group.They were divided into the self-made Buqi Huoxue decoction combined with heat sensitive moxibustion treatment group(TCM treatment group)and the Western medicine control group.The main symptom scores,TCD(brain color Doppler)test results and clinical efficacy of patients in each treatment group before and after treatment were statistically analyzed.Results After treatment,the scores of vertigo,head,neck and shoulder pain,tinnitus,hearing loss,dizziness,nausea and vomiting,cataplexy,amnesia and insomnia in the TCM treatment group were all lower than those in the Western medicine control group(P<0.05).The blood flow velocity of basilar artery,left vertebral artery and right vertebral artery,peak systolic blood velocity and vascular pulse index were all higher than those in the Western medicine control group(P<0.05),and the vascular resistance index was lower than that in the Western medicine control group Control group(P<0.05).The total effective rate of TCM treatment group was 93.33%(28/30),which was higher than that of Western control group(70.00%,21/30)(P<0.05).Con⁃clusion The clinical effect of self-made Buqi Huoxue decoction combined with heat sensitive moxibustion in the treatment of cervical spondylosis of vertebral artery type is better than that of Western medicine,which can effectively reduce the symptoms of patients,im⁃prove the signs and cerebral hemodynamics of patients,and enhance the clinical effect.
