参芪平律方联合体外反搏治疗射血分数降低心力衰竭的临床研究

摘　　要：目的观察参芪平律方联合体外反搏治疗射血分数降低心力衰竭(EFrHF)患者的临床疗效。方法选取2022年2月—2023年6月在河北医科大学第一医院治疗的60例EFrHF气虚血瘀证患者,随机分为对照组和试验组各30例。2组均给予西医标准化治疗,在此基础上,对照组患者给予体外反搏治疗4周,试验组患者给予体外反搏联合参芪平律方口服4周。观察2组治疗前后气虚血瘀证评分、心功能指标[左室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、血浆氨基末端脑钠肽前体(NT-proBNP)]、生活质量[6 min步行距离(6MWT)]的变化。结果与治疗前比较,治疗4周后2组患者气虚血瘀证评分均明显降低,心功能指标均明显改善,6MWT均明显延长,且试验组患者上述指标改善情况均明显优于对照组,差异均有统计学意义(P均<0.05)。结论参芪平律方联合体外反搏治疗能够显著减轻EFrHF气虚血瘀证患者临床症状,改善心功能,提高生活质量。Objective It is to observe the clinical efficacy of Shen Qi Pingluo decoction combined with external counterpulsation in the treatment of patients with heart failure with reduced ejection fraction(EFrHF).Methods Sixty patients with EFrHF and syndrome of Qi deficiency and blood stasis treated in the First Hospital of Hebei Medical University from February 2022 to June 2023 were randomly divided into 30 patients in the control group and 30 patients in the experimental group.Both groups were treated with western medicine standardized therapy.Additionally,the patients in the control group were treated with extracorporeal counterpulsation for 4 weeks,and the patients in the experimental group were treated with extracorporeal counterpulsation combined with Shen Qi Pingluo decoction for 4 weeks.The changes in the scores of syndrome of Qi deficiency and blood stasis,cardiac function indexes(left ventricular ejection fraction(LVEF),left ventricular end-systolic internal diameter(LVESD),left ventricular end-diastolic internal diameter(LVEDD),plasma amino-terminal brain natriuretic peptide precursor(NT-proBNP)),and the quality of life(6 min walking test(6MWT))before and after the treatments were observed in the two groups.Results Compared with before treatment,the scores of syndrome of Qi deficiency and blood stasis were significantly decreased,the cardiac function indexes were significantly improved,and the 6MWT was significantly prolonged in the patients of the two groups after 4 weeks of treatment,and the improvements of the above indexes of the patients in the experimental group were significantly better than those of patients in the control group,and the differences were all statistically significant(all P<0.05).Conclusion Shen Qi Pingluo decoction combined with external counterpulsation can significantly alleviate the clinical symptoms,improve cardiac function,and improve the quality of life of patients with EFrHF with syndrome of Qi deficiency and blood stasis.
